老年高血压伴糖尿病患者合理用药的选择与警示

摘　　要：目的促进老年高血压伴糖尿病患者的合理用药。方法检索文献并结合临床实践总结老年高血压伴糖尿病患者的用药不良反应、禁忌证与注意事项,总结合理用药的经验与教训。结果与结论老年高血压伴糖尿病患者应个体化选用降压药,如血管紧张素转换酶抑制剂(ACEI)和或血管紧张素Ⅱ受体拮抗剂(ARB)、钙拮抗剂(CCB)等,单用或联用如二甲双胍或磺酰脲类、胰岛素制剂等加控制血糖水平药物,药物品种总数应控制在5种以内,同时应坚持均衡营养、生活方式干预和适度运动。这样既可减少药品不良反应,又可使血压和血糖达标。Objective To promote the rational drug use in elderly patients with hypertension complicating diabetes. Methods By retrieveing the literatures and combining with clinical practice,the adverse drug reactions,contraindications and matters needing attention in the elderly patients with hypertension complicating diabetes were summarized. The experiences and lessons of the rational drug use were also summarized. Results and Conclusion Elderly patients with hypertension and diabetes should select the hypotensive drugs by individualization such as ACEI and / or ARB,CCB,etc.,single use or combination use of metformin or sulfonylurea,insulin preparations. The total number of drug varieties should be controlled within 5 varieties. Meanwhile,the balanced nutrition,lifestyle intervention and moderate exercise should be insisted,which can reduce the adverse drug reactions and also make the blood sugar and blood pressure to reach the standard.
