早期导泻对急性重症胰腺炎治疗作用分析

摘　　要：[目的]探讨不同时间导泻在急性重症胰腺炎（ASP）治疗中的应用价值。[方法]回顾分析我院于2011年8月～2013年8月收治的72例患者的临床资料,分为早期导泻治疗（发病48h内）36例和非早期导泻治疗（发病48h后）36例2组,观察2组腹痛和腹胀缓解时间、腹部压痛消失时间、血淀粉酶恢复正常的时间、发热消退时间、并发症发生率（为早期及后期并发症发生的总例数）、病死率、住院时间及住院费用等方面的差别。[结果]早期导泻治疗组腹痛和腹胀缓解时间、腹部压痛消失时间、血淀粉酶恢复正常的时间、发热消退时间、并发症发生率、住院时间及住院费用优于非早期导泻治疗组（P〈0.05）,但2组病死率差异无统计学意义。[结论]早期导泻治疗有利于较早地缓解腹部症状,缩短住院时间,减少住院费用,有利于ASP的恢复,值得临床推广。[Objective]To discuss the values of applying catharsis in the treatment of severe acute pancreatitis in different time.[Methods]Data from 72 patients in our hospital from Aug 2011 to Aug 2013 were analyzed retrospectively and divided into early catharsis group（within 48 hafter attack）and non-early catharsis group（48hafter attack）,36 cases in each group.The time of stomachache,stomachache relief,stomachache disappear and hemodiastase back to normal,fever reducing,complication incidence（total cases in early and late stage）,mortality,hospital time and cost of the both groups were evaluated and compared.[Results]The time of stomachache,stomachache relief,stomachache disappear and hemodiastase back to normal,fever reducing,complication incidence,hospital time and cost of early catharsis group were better than non-early catharsis group（P〈0.05）,but the difference of mortality between 2groups had no significance（P〉0.05）.[Conclusion]Early catharsis can relieve symptoms of stomachache early,shorten hospital time,reduce hospital cost and is more helpful to the recovery of ASP.
