脓毒症患者血磷水平与疾病严重程度及预后的相关性研究

摘　　要：目的：探讨血磷浓度与脓毒症患者疾病严重程度及预后的相关性。方法：选择2012-06-2014-05我院重症医学科共收治脓毒症患者147例,选取其中存在低磷血症的62例（血磷浓度≤0.8mmol/L）患者做回顾性研究,并根据血磷水平分为2组,A组：低中度低磷组（血磷浓度0.8~0.30mmol/L）;B组：重度低磷组（血磷浓度〈0.30mmol/L）,比较2组患者APACHEⅡ评分及病死率,同时根据预后分为存活组及死亡组,比较2组性别、血磷浓度、年龄、降钙素原（PCT）等因素。结果：A、B 2组APACHEⅡ评分及病死率比较差异有统计学意义（P〈0.05）;死亡组血磷明显低于存活组（P〈0.05）,而存活组与死亡组年龄、性别比较差异无统计学意义,死亡组降钙素原高于存活组（P〈0.05）。结论：脓毒症患者血磷浓度越低,病情越重,预后越差。血磷水平可作为判断脓毒症患者预后的指标之一。Objective：To investigate the relationship between hypophosphatemia,the severity and the prognosis of disease in patients with sepsis.Method：147patients with sepsis were treated in Intensive Care Unit,affiliated hospital of Weifang medical university during Jun 2012 to May 2014.62 cases of them had hypophosphatemia（serum phosphate concentration ≤0.8 mmol/L）.Give retrospective study to the 62 cases and divid them into two groups according to serum phosphate levels：Group A：Low and moderate group（Serum phosphorus0.30mmol/L）;Group B：Severe phosphorus group（Serum phosphorus≤0.30mmol/L）.Compare the score of acute physiology and chronic health evaluationⅡ（APACHEⅡ）and mortality between the two groups.They were also divided into two groups accoreding to the prognosis：survival group（n=40）and death group（n=22）.Compare the differences of gender,serum phosphorus,age and procalcitonin.Result：The score of APACHE Ⅱand mortality was statistically significant different between the group A and group B（P〈0.05）;the serum phosphorus of death group was significantly lower than the survival group（P〈0.05）.The difference of age,gender between the survival and death group was not statistically significant（P〈0.05）,procalcitonin of the death group was higher than that of the survival group（P〈0.05）.Conclusion：The lower the phosphorus in patients with sepsis,the more severe the disease,the worse the prognosis.The level of serum phosphorus may be used as an indicator to determine the prognosis of patients with sepsis.
