基于网络医疗平台健康管理对肿瘤化疗间歇期PICC带管患者的影响研究

摘　　要：目的探讨基于网络医疗平台健康管理对肿瘤化疗间歇期PICC带管患者的影响。方法选取2013年12月-2014年6月新疆某三级甲等医院肿瘤化疗间歇期PICC置管患者138例,按照随机数字表将患者分为试验组和对照组。试验组由PICC健康管理小组建立电子健康档案,基于网络医疗平台,应用电话、短信、QQ群和PICC园地论坛等管理方式进行健康干预;对照组按照传统的健康教育模式进行每月一次的电话随访。健康管理3个月后对两组患者并发症发生率、非计划拔管率和患者家庭功能得分进行统计学分析。结果两组患者血栓发生率比较差异无统计学意义(P>0.05),试验组患者静脉炎、导管感染、导管堵塞、导管渗漏、导管脱出率和非计划性拔管率低于对照组,差异有统计学意义(P<0.05);患者的家庭功能得分也高于对照组(P<0.05)。结论肿瘤化疗间歇期PICC带管患者基于网络医疗平台健康管理的实施,可以显著降低患者导管相关并发症的发生和非计划拔管率的发生,促进患者家庭健康,提高患者导管自我管理能力。Objective To discuss effect of health management based on the network medical platform for patient with PICC during intermittent chemotherapy .Method 138 catheterization patients in a three level of first‐class hospi‐tal in Xinjiang during December 2013 to June 2014 was selected and divided into experimental and control group ac‐cording to random number table .PICC health management team established electronic health records for experimen‐tal group ,management style based on the network medical platform ,such as the telephone ,SMS ,QQ group and PICC campus BBS ,etc for health intervention .According to the traditional health education model ,the control group conducted telephone follow‐up once a month ,the incidence of complications in two groups ,unplanned extuba‐tion and patients with family function was selected for statistical analysis .Result There was no significant difference in incidence of thrombosis between two groups（P〉0 .05） Patients with phlebitis in experimental group was much lower than that of he control group .The rate for catheter infections ,thrombosis ,catheter blockage ,catheter frac‐ture ,catheter extrusion and unplanned extubation was significant difference （P〈0 .05） .The patient＇s family func‐tion were also higher than that of control group （P〈0 .05） .Conclusion The implementation of health management based on the network medical platform for patients with PICC during intermittent chemotherapy can significantly re‐duce catheter‐related complications and unplanned extubation ,to enhance patients healthy family and improve the catheter self‐management ability .
