摘　　要：目的探讨ICU需机械通气的慢性阻塞性肺疾病(慢阻肺)呼吸衰竭患者心理状况及多虑平联合心理干预的临床疗效。方法选择2009年6月—2014年12月入住河北联合大学附属医院ICU需机械通气的150例慢阻肺呼吸衰竭患者,进行焦虑自评量表(SAS)、抑郁自评量表(SDS)评分,了解其心理状况;给予患者多虑平联合心理干预,比较干预前后患者SAS、SDS评分。结果患者干预后SAS、SDS评分均较干预前改善,轻、中度组干预前后SAS、SDS评分比较,差异有统计学意义(P<0.05)。结论及早识别ICU患者焦虑、抑郁情绪,早期给予多虑平联合心理干预可改善患者不良心理状况,减轻焦虑、抑郁情绪对患者恢复造成的影响,促进患者康复。Objective To observe the psychological status of chronic obstructive pulmonary disease（ COPD） patients complicated with respiratory failure in ICU who need mechanical ventilation and the effect of doxepin combined with psychological nursing. Methods A total of 150 COPD patients complicated with respiratory failure in ICU who need mechanical ventilation were chosen in the Affiliated Hospital of Hebei United University from June 2009 to December 2014. Self-rating Anxiety Scale（ SAS）,Self-rating Depression Scale（ SDS） was used to evaluate the psychological status of all patients,and all of them were treated with doxepin combined with psychological nursing. SAS and SDS scores were compared before and after intervention.Results After intervention,SAS and SDS scores of the patients were improved compared with those before intervention; there were significant differences of SAS and SDS scores between mild group and moderate groups before and after intervention（ P 〈0. 05）. Conclusion Early identification of anxiety and depression of COPD patients complicated with respiratory failure in ICU who need mechanical ventilation and early giving doxepin combined with psychological nursing can improve the adverse psychological status,reduce anxiety and depression,is helpful to the recovery.
