降钙素原对烧伤脓毒症诊断价值的Meta分析

摘　　要：目的系统性评价降钙素原对烧伤脓毒症的诊断价值。方法检索Pub Med、Elsevier、Springer、Cochrane图书馆、CNKI、万方数据库中的文献,根据QUADAS评价文献的质量。用Meta-Disc1.4和Stata12.0软件进行统计量的计算及图表绘制。结果纳入文献8篇,分两组:组1合并统计393例烧伤患者,汇总敏感度、特异度和SROC AUC、Q*值分别为0.88、0.82和0.930 7、0.866 0,组2合并统计105例烧伤患者105例共1351个时间点的数据,汇总敏感度、特异度和SROC AUC、Q*值分别为0.70、0.82和0.901 8、0.833 1,诊断阈值是异质性的主要来源。结论PCT对烧伤脓毒症诊断效能较高,临床可应用该指标早期诊断烧伤脓毒症,同时需要联合其他诊断指标,进一步提高烧伤脓毒症诊断的敏感度和特异度。Objective To systematic evaluate the value of the procalcitonin test for diagnosing burns sepsis. Methods A search in PubMed, Elsevier, Springer, Cochrane Library, CNKI, Wanfang Chinese Periodical Database was performed to i- dentify relevant articles. Statisticle analysis was performed by employing Meta-Disc 1.4 and Stats 12.0 software. Results 8 articles were included and divided into two groupes. 393 cases of bum patients were included in group 1, and the pooled sensi- tivity ,specificity and SROC AUC ,Q^＊ index were 0. 88,0.82 and 0. 9307,0. 8660. 105 cases of burn patients were included in group 2, which had1351 time points data, and the pooled sensitivity, specificity and SROC AUC, Q^＊ index were 0.70,0.82 and 0. 901 8,0.8331. The diagnostic threshold is the main source of heterogeneity. Conclusion PCT test showedgood accuracy in diagnosing burns sepsis, and this index can apply into the early diagnosis of sepsis in bum patients, as well as PCT should be combined with other diagnostic markers to further improve the sensitivity and accuracy in the diagnosis of sepsis.
