3517例门诊浅部真菌病发病及镜检结果分析

摘　　要：目的了解近年来延安地区皮肤科门诊浅部真菌病发病的种类、构成比及流行病学特点,评估直接镜检法对浅部真菌病的诊断意义。方法采用回顾性分析的方法,对3 517例临床拟诊为浅部真菌病患者标本进行直接镜检,并对数据进行统计分析。结果近5年来浅部真菌病的发病率呈上升趋势,各种浅部真菌病发病种类和构成比没有明显变化。浅部真菌病镜检阳性率排在前3位的是足癣(58.22%)、甲癣(52.39%)、手癣(51.62%),总阳性率为50.46%;各年龄组中镜检阳性率最高为中青年组(51.92%),其次为老年组(47.80%),最后为婴儿及少儿组(46.44%);男性头癣镜检阳性率低于女性,差异具有统计学意义(P<0.05)。其他浅部真菌病男性镜检阳性率均高于女性。结论延安地区浅部真菌发病的种类和构成比无明显变化,直接镜检法作为真菌学检验方法经济、简便易行,值得临床使用。Objective To investigate the variety and constitute of superficial mycoses,as well as epidemiological characteristics in dermatology clinic in Yan＇an To evaluate the significance of microscope examination in the diagnosis of superficial mycosis.Methods Direct microscopy was performed on clinical specimens collected from 3 517 cases of suspected patients with superficial mycosis in our hospital,and analyzed by retrospective survey. Results The incidence of superficial mycosis had been on the rise. There was no obvious change in variety and constitute of superficial mycoses. The positive rate under microscopy on the top list was： tinea pedis（ 58. 22%）,tinea unguium（ 52. 39%） and tinea manus（ 51. 62%）,with the overall positive rate of50. 46%. The positive rate of young and middle- aged group was the highest（ 51. 92%）,secondly the elderly group（ 47. 80%）,the last infants and children＇s group（ 46. 44%）. The microscopy positive rate of male tinea capitis was lower than women,the difference was statistically significant（ P〈0. 05）. The positive rate of men was higher than women in other superficial fungal diseases. Conclusion There was no obvious change in variety and constitute of superficial mycosis in dermatology clinic in Yan＇an. Direct microscopic examination was worth using clinically as an economic and convenient method for diagnosis of superficial mycosis.
