跨理论模型在社区2型糖尿病患者自我管理中的应用

摘　　要：目的:探讨跨理论模型结合团体咨询的技巧对社区2型糖尿病患者空腹血糖、糖化血红蛋白及生命质量的影响。方法:将80例2型糖尿病患者随机分为对照组和干预组各40例。对照组实施常规社区健康教育,干预组根据跨理论模型的不同阶段建立自我管理小组采用团体咨询的技巧,均干预6个月,比较2组干预效果。结果:2组患者干预6个月后的空腹血糖及糖化血红蛋白均下降,且干预组明显优于对照组(P<0.05);2组患者生活质量均有提高,且干预组提高程度明显高于对照组(P<0.05)。结论:应用跨理论模型结合团体咨询的技巧能有效提高2型糖尿病患者的生活质量,促进血糖控制。[Objective]To discuss the application effect of The Transtheoretical Model of Change on fasting blood glucose in patients with type 2 diabetes in community,glycosylated hemoglobin and the quality of life.[Method]80 cases of type 2 diabetic patients were randomly divided into intervention group and control group with40 cases in each.The control group received the routine health education,intervention group according to different stages of the transtheoretical model of self management group by group counseling skills,were intervention for 6months,compared with two groups of intervention effect.[Result]the two groups of intervention in patients with blood glucose and HbA1 c fasting after 6 months was decreased,and the intervention group was significantly better than the control group（P〈0.05）;two patients quality of life were improved,and the intervention group increased significantly than that in the control group（P〈0.05）.[Conclusion]the application of group counseling skills combined with the step of trans theoretical model as the theoretical basis of self management,can effectively improve the quality of life and blood glucose control in patients with type 2 diabetes has a significant role in promoting.
