摘 要:目的:探讨高原地区不同生活环境下藏族人群血脂代谢特点及与对氧磷酶1(PON1)及氧化低密度脂蛋白(ox-LDL)变化的关系。方法:对青海省玉树县结古镇(城镇地区,平均海拔3 700m)和隆宝镇(纯牧业地区,平均海拔4 800m)176例藏族人群进行脂代谢的特点及与PON1、ox-LDL的关系进行研究分析。结果:1玉树地区高脂血症患病率为46.0%(81/176例),其中结古镇组高脂血症的患病率为60.2%(71/118例),显著高于隆宝镇组17.2%(10/58例);结古镇组高TC血症、高TG血症、高LDL血症的患病率分别为44.9%、25.4%和47.5%,显著高于隆宝镇组19.0%、5.2%和17.2%),P<0.01;2相关分析显示PON1与TC、TG呈负相关,ox-LDL与海拔及Hb呈正相关,P<0.01;3Logistic回归分析显示,影响脂质代谢的主要因素为年龄、住址(结古镇和隆宝镇)、Hb及腹围。结论:不同海拔不同生活环境藏族血脂水平与PON1及ox-LDL浓度不同,血脂增高是血浆PON1降低原因之一,海拔的差异性、城镇化生活环境、高龄及血红蛋白可能是高原地区藏族PON1及ox-LDL水平不同的一个影响因素。Objective: To investigate the characteristics of lipid metabolism and its relationship with paraoxonase 1(PON1) and low- density lipoprotein( ox- LDL) in Tibetans at the different altitude. Methods: A total of 118 Tibetans( aged 55.7 ± 12.9 years) residing at 3 700 m and 58 Tibetans( aged 55.33 ± 9.45) residing at 4 800 m in Yushu area were selected in this study. According to the 2007 'China Adult Dyslipidemia Prevention Guideline',the plasma levels of PON1,ox- LDL,cholesterol( TC),triglycerides( TG),low- density lipoprotein cholesterol( LDL- C),and high-density lipoprotein cholesterol( HDL- C) were measured by ELISA assay at two different altitudes mentioned above. Results: TC,TG,and LDL- C values were( 185. 18 ± 64. 38) mg / d L,( 118. 36 ± 95. 40) mg / d Land( 116. 53 ± 53. 50) mg / d L,respectively in Jiegu Town Group. And these were significantly higher than those in Longbao Town Group( 142. 69 ±59. 62) mg / d L,( 52. 99 ± 51. 87) mg / d L and( 81. 49 ± 49. 75) mg / d L,respectively,( P〈 0. 001). No significant difference was found in the HDL- C levels of both groups. The rate of hyperlipidemia in Jiegu group( 59. 2%) was significantly higher than Longbao( 22. 2%) group( P〈0.01). The hyperlipidemia involved TC,TG and LDL- C in Jiegu group were 44. 9%,25. 4% and 47. 5%,respectively,and 19. 0%,5. 2%,17. 2%,respectively,in Longbao group.Conclusion: Urbanized living environment of pastoral Tibetans is an important factor of increased blood lipids in the high altitude area. Hypoxic environment may be a factor to lipid oxidation in Tibetan people on the Plateau.
