摘  要:目的研究台州地区人群血清单核细胞趋化蛋白-1(MCP-1)水平及MCP-1-2518A/G多态性与冠状动脉粥样硬化(atherosclerosis,AS)的关系,为AS的防治提供科学依据。方法收集2010年7月至2012年6月在浙江省台州医院行冠状动脉造影的住院患者共358例,根据冠脉造影结果按狭窄程度分为不同病变组及对照组,用Gensini评分系统评价冠状动脉损害严重程度,采用酶联免疫吸附法测定血清MCP-1水平,用多聚酶链反应-限制性片段长度多态性(PCRRFLP)法分析其基因2518A/G位点的多态性,所有生化指标采用雅培全自动生化分析仪进行检测。结果冠心病组(n=260)吸烟史、高血压史的比例高于对照组(n=98),差异有统计学意义(P<0.05)。冠心病组血清C反应蛋白(CRP)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、血糖(Glu)、乳酸脱氢酶(LDH)、乳酸脱氢酶同工酶1(LDH1)、肌酸激酶(CK)及肌酸激酶同工酶(CK-MB)水平与对照组比较,差异均有统计学意义(P<0.05)。冠心病组血清MCP-1水平[(242.47±45.97)pg/ml]高于对照组[(221.12±57.10)pg/ml],差异有统计学意义(P<0.01)。急性心肌梗死(AMI)组(n=55)与不稳定型心绞痛(UAP)组(n=116)的MCP-1水平明显高于对照组,差异均有统计学意义(P<0.05)。MCP-1-2518A/G位点多态性与AS无明显相关性。血清MCP-1水平与Gensini积分呈正相关(r=0.140,P<0.05),基因型分布符合Hardy-Weinberg平衡。结论在台州地区人群中,MCP-1-2518A/G位点多态性与AS的发生无明显相关性,但血清MCP-1水平与AS严重程度有关,并反映了病情的严重程度,可以作为初步预测急性冠脉事件和判断AS严重程度的参考指标。Objective To investigate the relationship between the coronary artery atherosclerosis(AS) and the monocyte chemoattractant protein(MCP-1) level or gene polymorphisms in patients with coronary artery disease of Taizhou. Methods From July2010 to June 2012, 358 in-patients with coronary angiography of Taizhou hospital served as the subjects. According to the results of coronary angiography, the subjects were divided into control group and groups with different lesions. The coronary lesions were evaluated by Gensini coronary scoring system, ELISA was used to detect the levels of serum MCP-1, PCR-RFLP test was utilized to analyze the MCP-1 polymorphisms( 2518 A / G site) and all biochemical parameters were tested by Abbott automatic biochemical analyzer. Results The smoking rate and hypertension rate in coronary heart disease group(260 cases) were significantly higher than those in control group(98 cases)(P〈0.05). The serum CRP, TG, HDL-C, Glu, LDH, LDH1, CK and CK-MB levels in coronary heart disease group were significantly higher than those in control group(P〈0.05). The serum MCP-1level [(242.4±45.97) pg/ml] in coronary heart disease group were significantly higher than that [(221.12±57.10) pg/ml] in control group(P〈0.05). The levels of MCP-1 in acute myocardial infarction(AMI) sub-group(55 cases) and unstable angina pectoris(UAP) sub-group(116 cases) were significantly higher than those in stable angina pectoris(SAP) sub-group(89 cases) and control group(P〈0.05). MCP-1-2518 A/G gene polymorphisms were not associated with AS. Serum MCP-1 level was positively correlated with Gensini score(r=0.140, P〈0.05). Genotype distribution accorded with Hardy-Weinberg balance. Conclusion In Taizhou population, no relationship between MCP-1-2518 A/G and AS is observed, but serum MCP-1 level is associated with the extent of coronary artery atherosclerosis.
