量化定位角度牵引下神经根型颈椎病体感诱发电位变化与疗效的临床观察

摘　　要：目的观察量化定位角度牵引治疗神经根型颈椎病的疗效与皮层诱发电位（cerebral evoked potential CEP）变化的相关性。方法全部60例患者随机分为两组,对照组选用中立位0度牵引,观察组采用量化定位角度牵引,后者根据临床表现并对照颈椎X线片（少数病例为CT或MRI片）而确定牵引角度。牵引重量为体重的10%∽15%,牵引时间30 min。1次/d,治疗10次后进行统计。治疗前后用肌电图机各记录皮层诱发电位一次。结果两组内治疗前后疼痛积分、颈椎曲度及CEP潜伏期值均有显著性差异,有统计学意义。两组间治疗后疼痛积分及CEP潜伏期值对比,经统计学处理有显著性差异,且观察组疗效优于对照组。两组间颈曲值治疗后相比较,差异不大,无明显统计学意义。结论量化定位角度牵引治疗神经根型颈椎病疗效显著,且患者的疗效与CEP潜伏期值的变化成正相关,对临床治疗的疗效评定具有客观的指导意义。Objective To observe the curative effect and correlation of the cerebral evoked potential（ CEP） changes on of nerve root which was treated with quantitative orientation angle traction. Methods 60 patients with cervical spondylosis were randomly divided into 2 groups. The control group were treated by neutral zero traction while the observation group with quantitative orientation angle traction,the traction angles for the latter based the clinical manifestations and their corrisponding cervical images. The strength was 10 to 15% of the patien＇s body weights and it lasted 30 minutes once for ten days. Statistics and data analysis was done after 10 times. Before and after treatment,evoked cortical potential was recorded respectively with electromyography machines. Results There was a significant difference in pain points,cervical flexure and CEP incubation before and after the treatment,so was the value of pain points in contrast with CEP latency by means of statistics analysis after the treatment. The curative effect of the observed group was better than that of the contol one. Neck curve between the two groups has no statistical significance before and after. Conclusion The curative effect on cervical spondylosis of nerve root by quantitative angle traction is closely relative to the change of CEP incubation period value,which was of objective significance to the evaluation of curative effect in clinic.
