六君子汤加减治疗慢性阻塞性肺疾病稳定期患者呼吸肌疲劳临床观察

摘　　要：目的:观察六君子汤加减对慢性阻塞性肺疾病(COPD)稳定期患者呼吸肌疲劳及肺功能的改善作用。方法:92例COPD稳定期患者随机分成六君子汤治疗组47例和对照组45例,对照组给予西医常规治疗,治疗组在对照组常规治疗基础上,给予中药六君子汤加减煎服。观察肺功能及呼吸肌功能:第1秒用力呼气容积占预计值百分比(FEV1%)、第1秒用力呼气容积与用力肺活量比值(FEV1/FVC),最大吸气压(MIP)。结果:2组临床疗效比较,差异有显著性意义(P<0.05),治疗组疗效明显优于对照组。治疗前2组FEV1%、FEV1/FVC%、MIP比较,差异均无显著性意义(P>0.05)。对照组治疗后FEV1%、MIP、FEV1/FVC%无明显变化(P>0.05)。治疗组治疗后FEV1%、FEV1/FVC%、MIP均有明显改善,差异有显著性意义(P<0.05);与对照组比较,治疗组治疗后FEV1%、FEV1/FVC%、MIP变化明显,差异有显著性意义(P<0.05)。结论:六君子汤能有效缓解COPD患者呼吸肌疲劳,改善肺通气功能,对治疗COPD缓解期患者有良好的疗效。Objective： To observe the effect of modified Liujunzi decoction on improving respiratory muscle tatigue and lung function of stable phase patients with chronic obstructive pulmonary disease（COPD）. Methods： Ninety-two stable phase COPD patients were randomly divided into treatment group （47 cases） and control group（45 cases）. The control group received western medicine conventional treatment, and the treatment group was given modified Liujunzi decoction on the basis of treatment for the control group. Lung function indexes including forced expiratory volume in first second（FEV1%）, and the ratio of FEV1% to forced vital capacity（FVC）, maximal inspiratory pressure（MIP） were measured before treatment and 2 months after treatment. Results： The clinical effect in the treatment group was superior to that in the control group（P 〈 0.05）. Beforetreatment, the differences of FEV1%, FEV1/FVC% and MIP were insignificant between the two groups（P 〉0.05）. After treatment, the levels of FEV1%, FEV1/FVC%, MIP had no statistically significant change in the control group（P〉 0.05）, while were markedly improved in the treatment group after treatment（P 〈 0.05 compared with those before treatment and those in the control group after treatment）, and were superior to those in the control group after treatment. Conclusion： Liujunzi decoction has obvious effect for the treatment of stable phase COPD by relieving respiratory muscle fatigue and improving lung function.
