六安市HIV感染夫妻间性传播影响因素分析

摘　　要：目的了解夫妻间HIV性传播率和影响因素,为制定防治措施提供科学依据。方法采用回顾性研究方法,选择有稳定婚姻的HIV感染者/病人及其配偶作为调查对象。结果 HIV夫妻间传播率为30.0%(39/130),男传女性传播率为33.78%(25/74),女传男性传播率为25.0%(14/56)。单因素分析显示:夫妻性行为频次(χ2=19.433,P=0.000)、夫妻任一方性病史(χ2=4.288,P=0.038)是夫妻间HIV性传播的影响因素。双阳组先证病例确证时CD4细胞计数均值低于单阳组(t=-2.47,P=0.015),感染与确证时间间隔长于单阳组(t=2.507,P=0.013)。多因素分析显示:先证病例性别、确证时年龄、夫妻性生活频次、感染与确证时间间隔是HIV夫妻间性传播的独立影响因素。结论六安市HIV夫妻传播率较高,与先证病例性别、确证时年龄、夫妻性生活频次、感染时间与HIV抗体阳性确证时间间隔等因素有关。Objective To investigate the sexual transmission rate of human immunodeficiency virus and its influencing factors between husbands and wives,and to provide scientific evidence for formulating AIDS prevention and control measures.Methods A retrospective study was conducted among HIV infected persons with stable marriages and their spouses.Results The transmission rate of HIV in the spouses was 30. 0%（ 39 /130）. The male-to-female and female-to-male sexual transmission rates of HIV were 33. 78%（ 25 /74） and 25. 0%（ 14 /56）,respectively. Univariate analysis showed that sexual behavior frequency（ χ2= 19. 433,P = 0. 000） and STD history of husband or wife（ χ2= 4. 288,P = 0.038） were the factors influencing HIV infection between husband and wife. The average CD4 cell count of the probands with HIV infection in the sero-discordant couple group was lower than that in the sero-cordant couple group（ t =-2. 47,P =0. 015）,and the interval from HIV infection to HIV confirmation was longer in the sero-discordant couple group than in the sero-cordant couple group（ t = 2. 507,P = 0. 013）. Multivariate analysis showed that sex,the age at HIV confirmation,the couple＇s sexual frequency and the interval from HIV infection to HIV confirmation were the independent factors influencing HIV sexual transmission between husband and wife. Conclusions The transmission rate of HIV between husband and wife in Liuan City is higher,and it is related to the proband＇s gender,the age at HIV confirmation,the couple＇s sexual frequency and the interval from HIV infection to HIV confirmation.
