摘  要:目的观察甲泼尼龙鼓室内注射与全身应用治疗不同分型特发性突发性感音神经性聋(ISSHL)的疗效。方法选取单耳特发性突聋患者167例,其中低中频下降型47例,中高频下降型42例,平坦型15例,全聋型63例。各型均采用随机数字表法分为2组,即全身给药组(84耳),鼓室给药组(83耳),对各型患者有效率及治愈率分组进行分析。结果鼓室内注射组低中频下降型治愈率较中高频下降型、全聋型高(P<0.05),鼓室内注射组低中频下降型治愈率较对照组高(P<0.05),其余三型间差异无统计学意义。结论低中频下降型ISSHL经鼓室内注射甲泼尼龙治疗比全身性给药效果更佳,其它三型如患者无激素应用禁忌建议首选全身用药。Objective To assess the therapeutic effect of intratympanic injection and systemic of methylprednisolone in treat- ment of typing idiopathic sudden sensorineural hearing loss (ISSNHL). Methods 167 patients with ISSNHL,including 47 with hearing loss in low- middle frequencies,42 in middle -high frequencies, 15 in all frequencies and 63 in total deafness,were randomly divided into two groups: the systemic drug group (84 ears) and intratympanic injection group (83 ears). Results In intratympanic injection group, the recovery rate in low - middle frequencies subgroups was significant higher than that in middle - high ( P 〈 0.05 ) and all fre- quencies subgroups ( P 〈 0.05 ). The patients of low - middle frequencies with intratympanie injection of methylprednisolone was signifi- cant higher than systemic application ( P 〈 0.05 ) , and there were no statistical significances in other three typing groups by intratympanic injection of methylprednisolone when compared with systemic application. Conclusion Intratympanie injection should be optimization for low - middle frequency type ISSHL. If patients were glucoeorticoids - free applications, systemic application should be applied for oth- er three typing frequencies.
