摘 要:目的探讨18F-脱氧葡萄糖正电子发射断层显像/计算机体层扫描(18F-FDG PET/CT)在不明原因发热(FUO)患者中筛查淋巴瘤的价值。方法回顾性分析41例不明原因发热患者的临床病历资料及18F-FDG PET/CT影像结果。结果 41例FUO患者中有13例PET/CT显像示淋巴瘤可能,经病理检查12例确诊为淋巴瘤。PET/CT结果对FUO病因淋巴瘤诊断的灵敏度、特异度、准确度、阳性预测值、阴性预测值分别为:100%(12/12)、96.2%(25/26)、97.4%(37/38)、92.3%(12/13)、100%(25/25);淋巴瘤组和非淋巴瘤组浅表淋巴结和深部淋巴结放射性增高例数比均无统计学差异,而两组的最大标准化摄取值(SUVmax)比较都具有显著性差异;两组瘤患者脾脏增大比率、放射性增高例数比率和SUVmax值比较均有显著性差异;两组全身多中心骨髓放射性增高例数比率的比较无显著性差异,SUVmax值的比较差异具有统计学意义。结论18F-FDG PET/CT可通过淋巴结、脾脏及全身多中心骨髓的SUVmax值为在FUO中筛查淋巴瘤患者提供重要参考价值。Objective To explore the value of 18F-FDG PET/CT in screening lymphoma of the FUO patients. Methods The clinical data and imaging results of 41 FUO patients were analyzed retrospectively,to explore the value of PET /CT in screening lymphoma of the FUO patients. Results Among the 41 FUO patients,13 cases were revealed lymphoma by PET /CT imaging,of which 12 cases were diagnosed as lymphoma by pathological examination. The sensitivity,specificity,accuracy,positive predictive value,and negative predictive value of lymphoma diagnosis of FUO patients with PET /CT were: 100%( 12 /12),96. 2%( 25 /26),97. 4%( 37 /38),92. 3%( 12 /13),100%( 25 /25),respectively. No statistical difference in increased radioactivity ratio of superficial and deep lymph nodes were found between lymphoma and non-lymphoma group,but there is significant difference in SUVmax value between the two groups patients. There is significant difference in the ratio of increased size of spleen and increased radioactivity and SUVmax value between the two groups. There is no significant difference in the ratio of increased whole body multi-center bone marrow radioactivity between the two groups. And there is significant difference in SUVmax value between the two groups. Conclusion 18F-FDG PET /CT imaging can provide important reference information in screening lymphoma of FUO patients by SUVmax value of lymph nodes、spleen and whole body multi-center bone marrow.
