小切口外侧入路与常规切口髋关节置换术治疗股骨颈骨折的疗效评价

摘　　要：目的观察小切口外侧入路与常规切口髋关节置换术治疗股骨颈骨折的临床疗效。方法将120例股骨颈骨折患者纳入研究,随机分为给予小切口外侧入路髋关节置换术的观察组和给予常规切口髋关节置换术的对照组。观察记录两组患者手术相关指标及远期关节功能情况。结果观察组患者患者手术时间稍短于对照组,住院总费用也低于对照组,但组间比较差异无统计意义(P>0.05)。观察组患者的术中出血量、术后下床活动时间、住院总时间均低于对照组,差异有统计学意义(P>0.05)。观察组患者的术后3个月、半年以及一年时Harris评分和HSS评分均明显高于对照组,差异有统计学意义(P<0.05)。结论小切口外侧入路髋关节置换术有助于减小手术创伤、促进术后恢复、改善关节功能,且手术时间与常规切口手术相当,是治疗股骨颈骨折的理想方式。Objective To study the effect of small incision with lateral approach and conventional incision for total hip arthroplasty in treatment of patients with femoral neck fracture. Methods Patients with femoral neck fracture were enrolled and randomly divided into observation group and control group,patients in observation group were given with small incision by lateral approach for total hip arthroplasty and patients in control group were given with conventional incision for total hip arthroplasty. Then operation related indexes and long- term joint function were observed. Results There was no difference between these two group in operating time and total cost for hospitalization（ P〈0. 05）. Volume of intraoperative bleeding,postoperative ambulation time and hospitalization time in observation group were less than those of control group,their difference was statistically significant（ P〈0. 05）. In 3,6 months and one year after the operation,Harris scores and HSS scores of patients in observation group were higher than those of control group,and their difference was statistically significant（ P〈0. 05）. Conclusion Small incision with lateral approach for total hip arthroplasty is an ideal way for treatment of femoral neck fracture,and it can reduce operational trauma,promote postoperative recovery,improve the function of joint and achieve equal operating time with conventional incision operation.
