剖宫产术切口愈合不良的危险因素及临床特征分析

摘　　要：目的探讨剖宫产切口愈合不良的发生率及相关危险因素、临床特征、处理及预防措施。方法回顾性分析该院9564例剖宫产产妇的临床资料,统计分析愈合不良发生率、影响因素、治疗及预防。结果发生切口愈合不良45例,发生率为0.47%;分别为脂肪液化12例、切口感染29例、裂开4例。经抗炎、清创、充分引流、二期缝合等治疗后均愈合。愈合不良的相关因素:肥胖、术前感染、术前贫血、胎膜早破、未预防应用抗生素,阴道检查过多等。结论剖宫产切口愈合不良的发生率大约为0.47%,涉及产妇自身、手术原因、手术过程等多方面因素,肥胖、术前感染、术前贫血、胎膜早破、未预防应用抗生素、阴道检查过多等是造成剖宫产术后切口愈合不良的主要独立因素。Objective To investigate the incidence of poor wound healing after cesarean section and its related risk factors,clinical characteristics and treatment and preventive measures.Methods The clinical data of 9,564 cases of puerperants underwent cesarean section in the Affiliated Shenzhen Nanshan Hospital of Guangdong Medical College from January,2006 to September,2012 were analyzed retrospectively.The incidence of poor wound healing,influencing factors,treatment and prevention were counted and analyzed.Results Among the 9,564 women surveyed,45（0.47%） developed poor wound healing after a cesarean section,included 12 cases of fat liquefaction,29 cases of surgical site infection,4 cases of incision dehiscence.After the treatment of antiinflammatory,debridement,adequate drainage and secondary suture,45 cases were all healed.Statistical analysis identified the following risk factors associated with poor wound healing,which included obesity,preoperative infection,preoperative anemia,premature rupture of membranes,lack of prophylactic antibiotics and excessive vaginal examinations.Conclusion The incidence of poor wound healing after cesarean section was approximately 0.47%,involving puerperant self,operation reason,operation process and other factors,obesity,preoperative infection,preoperative anemia,premature rupture of membranes,lack of prophylactic antibiotics and excessive vaginal examinations are the main independent risk factors of poor wound healing after cesarean section.
