摘 要:目的 比较首发精神分裂症患者糖脂代谢与健康人群的差异,探讨其与白细胞介素-1(IL-1β)及白介素-1受体拮抗剂(IL-1R)的关系.方法 入组符合ICD-10精神分裂症诊断标准的首发患者47例(病例组)及健康对照32人(对照组),收集一般人口学资料,测定空腹血糖、餐后血糖等代谢指标,血清IL-1β及IL-1 Rα水平,比较两组的差异;以是否患糖调节异常(IGR)分组,分析组间的IL-1β及IL-1 Rα水平差异.结果 两组的空腹血糖水平差异无统计学意义(P>0.05),但病例组餐后血糖较高(P<0.01).病例组血清IL-1β水平较对照组高(P<0.01).病例组糖调节异常(IGR)者12例,而对照组为0例;IL-1β及IL-1Rα水平在IGR患者、无IGR的患者及对照组间差异均有统计学意义(P<0.01).结论 首发的精神分裂症患者已存在糖代谢异常,该异常可能和血清IL-1β及IL-1Rα水平有关.Objective To compare glucose and lipid levels of first--episode schizophrenia patients with health control and to explore their relationship with interleukin-lbeta(IL-16) and interleukin-1 receptor antagonist α(IL-1Rα). Methods Forty-seven patients diagnosed with schizophrenia with the Tenth Revision of the International Statistical Classification of Diseases and Related Health Problems (ICD-10) and 32 health controls were enrolled. Demographic data was collected. Compared the difference between two groups in the fasting plasma glucose(FPG), post--prandial blood glucose(PBG)of 2 hours, plasma concentration of IL-16 and IL-1Rα. The concentration of IL-1β and IL-1Rα were also compared between groups with and without impaired glucose regulation(IGR). Results There was no significant difference in FPG between the two groups (P 〉 0.05). But PBG in patients group was higher (P 〈0.01). The level of IL-1β in patients was higher than the controls (P 〈 0.01). There were 12 people with IGR in patients group and none in control group. The concentrations of both IL-1β and IL -1Rα were significantly different between patients with IGR, patients without IGR and control group (P 〈 0.01). Conclusions There are significant glucose metabolic disorders in first first-episode schizophrenia patients and they may have relationship with the plasma concentration of IL-1β and IL- 1Rα.
