应用空心变距无头拉力螺钉(Acutrak螺钉)微创与Qwix螺钉切开手术治疗Jones骨折随机前瞻设计40例随访分析

摘　　要：目的探讨应用空心变距无头拉力螺钉（Acutrak螺钉）经皮微创与Qwix螺钉切开手术治疗Jones骨折的临床效果。方法采用随机、前瞻性对照研究的方法将2009年2月—2012年3月我科收治的40例Jones骨折患者分为无头螺钉（Acutrak）组20例,Qwix螺钉组20例,并评价两组手术时间,术后疼痛,AOFAS评分。结果 40例患者均获随访,随访时间11-23（17.2±5.5）个月。Acutrak组在术后1、3个月VAS评分明显低于Qwix组,在术后3个月AOFAS评分明显高于Qwix组,术后并发症及取出螺钉的病例数明显少于Qwix组。结论应用Acutrak螺钉微创技术治疗与传统切开手术比较,存在更少的术后疼痛、更好的患足功能、更少的并发症等优点。Objective To discuss the clinical effects of treatment for Jones fractures using percutaneous internal fixation with Acutrak screw versus open surgery with Qwix screw. Methods We prospectively analyzed 40 patients with Jones fractures admitted to our department from Feb 2009 to Mar 2013 and received percutaneous internal fixation with Acutrak screw（ n = 20） and open surgery with Qwix screw（ n = 20）. They were all evaluated by operation time,Visual Analogue Scale / Score,and AOFAS scoring system. Results All the 40 cases were followed up,with a mean period of 17. 2 ± 5. 5（ from 11 to 23） months. The mean VAS Score of Acutrak group at 1st and 3rd month postoperatively decreased significantly. The mean AOFAS Score of Acutrak group at 3 rd month postoperatively increased significantly. Postoperative complications and removal of hardware of Acutrak group significantly decreased. Conclusion The minimally invasive technique with Acutrak screw is superior to conventional open surgery in terms of less postoperative pain,better function of injured feet and decreased incidence of complications.
