江西省近5年孕产妇死亡变化趋势分析

摘　　要：目的:探讨江西省近5年孕产妇死亡相关因素的变化趋势和特点,为政府部门制定相关的政策提供科学依据。方法:在全省孕产妇死亡监测网内采用以人群为基础的流行病学调查方法。结果:近5年江西省孕产妇死亡率持续下降,下降幅度为34.9%;农村、城市孕产妇死亡率差距减小,2012年城市孕产妇死亡率(18.22/10万)超过农村(11.5/10万);产科出血(18.4%)仍是孕产妇死亡的主要原因;间接产科原因导致的孕产妇死亡比例上升,2012年间接产科原因(52.6%)超过直接产科原因(47.4%)。结论:关注城市孕产妇健康、加强产科出血的防治、提高产科医生识别和处理妊娠合并症的能力、加强产科与相关科室的协作、严格执行高危孕产妇分级管理及转诊制度等是降低孕产妇死亡率的突破点。Objective： To explore the change trend and characteristics of maternal death- related factors in the latest five years in Jiangxi province,provide a scientific evidence for the government developing relevant polices. Methods： A population- based epidemiological survey was conducted based on provincial maternal mortality surveillance network. Results： Maternal mortality in Jiangxi province in the latest five years decreased continuously,the decreasing range was 34. 9%; the gap of maternal mortality between rural areas and urban areas was gradually narrowing. In 2012,maternal mortality in urban areas was 18. 22 /100 000 live births,which was higher than that in rural areas（ 11. 5 /100 000 live births）. Obstetrical hemorrhage（ 18. 4%） was the main cause for maternal death; the percentage of maternal death induced by indirect obstetrical causes increased,the percentage in 2012（ 52. 6%） was higher than that of maternal death induced by direct obstetrical causes（ 47. 4%）. Conclusion： The breaking points of reducing maternal mortality include paying more attention to urban maternal health,strengthening prevention and treatment of obstetrical hemorrhage,improving the obstetricians＇ ability to identify and deal with pregnancy complications,strengthening the collaboration between obstetrical department and related departments,conducting classification management and transfer system of high risk pregnant women.
