摘 要:目的:探讨推广胎儿系统超声筛查的效果及意义。方法:收集湖北省内接受长期胎儿系统超声筛查培训后各级医院的反馈资料,包括医院年分娩人数、年系统超声检查人次、胎儿畸形检出例数及胎儿畸形病种及孕周分布。并与湖北省三网监测数据资料进行比较,分析胎儿畸形病种及检出孕周分布情况。结果:1接受胎儿系统超声筛查培训后各级医院的年分娩人数、年系统超声检查人次及胎儿畸形检出例数均呈不同程度的增长趋势。2三级医院、二级医院及安陆计划生育服务站胎儿畸形总超声检出率分别为1.26%、0.40%、0.34%。3接受胎儿系统超声筛查培训后医院孕28周以前检出的胎儿畸形占73.41%,明显高于湖北省三网监测显示的数据(11.26%)。4检出胎儿畸形病种排序发生变化、提高了严重、疑难畸形病种的检出率。结论:推广胎儿系统超声筛查可使湖北省内多数严重胎儿结构异常在28周以前得到诊断、避免出生,对降低出生缺陷的发生率,提高出生人口素质有重要的意义。Objective: To explore the effect and significance of popularizing and applying fetal system ultrasound screening. Methods: The feedback data from the hospitals in Hubei which accepted fetal system ultrasound screening training for a long time were collected,including the number of delivery per year,the number of pregnant women receiving prenatal ultrasound screening per year,the number of fetal malformations detected by prenatal ultrasound,the ranking of fetal malformations and gestational weeks. Then the data was compared with the monitoring network data of maternal mortality,child mortality and birth defects in Hubei,the ranking of fetal malformations and gestational weeks were analyzed. Results: After fetal system ultrasound screening training,the number of delivery per year,the number of pregnant women receiving prenatal ultrasound screening per year and fetal malformations detected by prenatal ultrasound increased. The total detection rates of fetal malformations by ultrasound were 1. 26%,0. 40% and 0. 34% respectively in tertiary- level hospitals,secondary- level hospitals and family planning service stations. After fetal system ultrasound screening training,73. 41% of fetal malformations were detected by ultrasound before 28 th gestational week,which was higher than network monitoring data( 11. 26%) in Hubei. The ranking of fetal malformations changed,the detection rates of serious and complicated fetal malformations were improved. Conclusion: Popularizing fetal system ultrasound screening can detect and diagnose most serious fetal malformations before 28 th gestational week and prevent serious birth defects,which has important significance in reducing the morbidity of birth defects and improving the quality of birth population.
