摘 要:目的检测高血压和非高血压患者在行冠脉CTA检查应用对比剂前后的血清胱抑素C(CysC)浓度,研究血清Cys-C对高血压和对比剂引起的肾脏早期损伤的评估。方法选取我院心内科行冠脉CTA检查者193例,将其分为高血压组97例以及对照组96例。应用乳胶颗粒增强免疫比浊法测定血清Cys-C浓度,肌氨酸氧化酶法测定SCr浓度。比较两组患者在应用对比剂前后血清Cys-C浓度、肾小球滤过率(eGFR)及血肌酐(SCr)浓度的差异。分析临床参数和血清Cys-C的关系。结果在应用对比剂前,高血压组血清Cys-C浓度明显高于对照组血清Cys-C浓度,而高血压组的eGFR则低于对照组。两组在应用对比剂后血清Cys-C浓度均明显升高,在高血压组eGFR在应用对比剂后明显降低。血清Cys-C浓度与eGFR负相关,而与糖尿病、冠心病、高血压、低密度脂蛋白、C反应蛋白正相关。结论血清Cys-C对高血压及对比剂引起的肾脏功能早期损伤的诊断比SCr更敏感。Objective To study the concentration of serum Cys-C in hypertension and non hypertension patients before and after the application of contrast agent to explore the value of Cys-C to the diagnosis of early renal injury caused by hypertension and contrast agent. Methods The individual-level cross-sectional data from 193 patients after the examination of computed tomography coronary angiography was selected in our department of the first hospital of China Medical University. All subjects are divided into hypertension group(96 cases) and control group(97 cases). Serum Cys-C concentration was determined by particles enhance nephelometry immune assay and SCr concentration was examined by enzymic creatinine assay method. The values of Cys-C, SCr and estimated glomerular fihration rate(eGFR) in two groups were analyzed before and after the application of contrast agent. The correlation of serum Cys-C with clinical parameters was analyzed. Results Before the application of contrast agent, concentration of serum Cys-C in hypertension group was obviously higher than that in control group, but lower for eGFR in hypertension than in control. After the application of contrast agent, the concentration of serum Cys-C was significantly increased in two groups. In hypertension group, the value of eGFR decreased obviously after the application of contrast agent. Serum Cys-C concentration was negatively correlated with the eGFR and positively correlated with diabetes, coronary heart disease, high blood pressure, low density lipoprotein cholesterol and C- reactive protein. Conclusion Serum Cys-C is more sensitive to the diagnosis of the early renal injury caused by hypertension and contrast agent than SCr.
