阿托伐他汀早期干预对合并高血压的2型糖尿病患者尿β_2微球蛋白与炎性因子的影响

摘　　要：目的:观察合并高血压的2型糖尿病患者尿β2微球蛋白(β2-MG)、血清尿素氮(BUN)、肌酐(SCr)水平变化与超敏C-反应蛋白(hs-CRP)、白细胞介素-6(IL-6)的关系及阿托伐他汀钙片早期干预的影响。方法:125例合并高血压的2型糖尿病患者随机分为他汀组(63例)和对照组(62例),对照组给予降糖、优化降压药物治疗,他汀组在对照组治疗基础上给予阿托伐他汀钙片20mg,每晚1次。分别于治疗前及治疗后6、12个月时检测尿β2-MG、BUN、SCr、hs-CRP、IL-6、糖化血红蛋白(HbAlc)及血脂水平。结果:两组患者治疗前的尿β2-MG、BUN、SCr、hs-CRP、IL-6、HbAlc及血脂水平均差异无统计学意义(均P>0.05),他汀组在治疗6、12个月时尿β2-MG、BUN、SCr、hs-CRP、IL-6、TC、LDL-C水平均明显下降,与治疗前比较差异有统计学意义(均P<0.05),与对照组治疗后比较差异亦有统计学意义(均P<0.05)。结论:对合并高血压的2型糖尿病患者给予阿托伐他汀早期干预治疗,不仅能调节患者血脂水平,而且在显著降低hs-CRP、IL-6等炎性因子水平的同时改善其尿β2-MG、BUN、SCr等肾功能指标。Objective：To explore the relationships and changes of urinary β2-microglobulin （β2-MG）, blood u- rea nitrogen （BUN）, serum creatinine （SCr）, high sensitive C-reactive protein （hs-CRP） and interleukin-6 （IL-6） in patients with type-2 diabetes mellitus combining hypertension, and observe the effects of atorvastatin early inter- vention. Method：One hundred and twenty-five patients with type-2 diabetes mellitus combining hypertension were randomly to conventional treatment group （n= 62） and atorvastatin group （20 mg/day, n= 63）. The investigative data included urinary β2-MG, BUN, SCr, hs-CRP, IL-6, HbAlc and blood lipids before and after 6,12 months of treatment in all patients. Result：There was no significant difference in urinary β2-MG, BUN, SCr, hs-CRP, IL-6, HbAlc and blood lipids in two groups before treatment （all P〉0.05）. Compared with control group and prethera- py,urinary β2-MG, BUN, SCr, hs-CRP, IL-6, TC, LDL-C levels were significantly decreased after 6 and 12 months of treatment in atorvastatin group （all P〈0.05）. Conclusion： Atorvastatin is not only able to further regu- late lipids profile （reduce TC and LDL-C levels）, but also significan-tly reduce the levels of inflammatory factors and improve renal function in patients with type-2 diabetes mellitus combining hypertension.
