摘  要:目的:研究胆固醇相关因子脂酰辅酶A:胆固醇酯酰转移酶1(ACAT1)、ATP结合盒转运蛋白A1(ABCA1)和炎性细胞因子TNF-α、IL-1β、MCP1、IL-10在老年退行性主动脉瓣疾病(DAVD)中的表达。方法:收集因退行性主动脉瓣狭窄而行手术切除的严重钙化主动脉瓣标本80例,经HE和免疫组织化学染色,观察退行性主动脉瓣组织学改变和ACAT1、ABCA1表达特点,RT-PCR检测组织中胆固醇相关基因CD68、ACAT1、ABCA1和炎性细胞因子TNF-α、IL-1β、MCP1、IL-10基因的表达,单元线性回归分析巨噬细胞标志分子CD68与ACAT1、ABCA1和TNF-α、IL-1β、MCP1、IL-10表达的相关性。结果:与正常主动脉瓣比较,DAVD增厚,结节性钙化灶存在,增生纤维组织排列紊乱使瓣膜失去原有结构,钙化周围发现巨噬细胞和其他炎性细胞、泡沫细胞、新生血管形成;免疫组织化学染色示:正常主动脉瓣未见巨噬细胞,而DAVD大量巨噬细胞浸润;RT-PCR检测显示,与正常主动脉瓣相比,DAVD中ACAT1和ABCA1的表达增高(P<0.05)。正常主动脉瓣无CD68表达,DAVD表达CD68。单元线性回归分析表明,CD68与ACAT1(r=-0.83,P<0.05),ABCA1(r=-0.89,P<0.05)表达正相关。与正常主动脉瓣相比,DAVD中TNF-α、IL-1β、MCP1的表达增高,而IL-10表达降低(P<0.05)。单元线性回归分析表明,CD68表达与TNF-α(r=0.87,P<0.05)、IL-1β(r=0.92,P<0.05)、MCP1(r=0.87,P<0.05)呈正相关,与IL-10(r=-0.64,P<0.05)负相关。结论:在DAVD病变中,巨噬细胞浸润的同时伴随着ABCA1、ACAT1和促炎因子表达增加,巨噬细胞标志CD68与胆固醇相关基因ABCA1、ACAT1和促炎因子正相关。Objective:To investigate the expression of the cholesterol related factor:acyl-CoA: cholesterol acyl- transferasesl (ACAT1),ATP binding cassette transporter A1 (ATP binding cassette transporter A1, ABCA1) and inflammatory cytokines TNF-α, IL-1β, MCP1, IL-10 in degenerative aortic valves disease (DAVD). Method: The aortic valves specimens were collected in 80 cases with DAVD, the histology characteristics and the expression of ACATI, ABCA1 in degenerative aortic valves were observed by HE and immunohistochemical staining. The expression of cholesterol related CD68, ACAT1, ABCA1 gene and inflammatory factor TNF-α, IL-1β, MCP1, IL-10 gene were detected by RT PCR, the correlation of the expression of molecular markers of macrophages CD68 with ABCA1, ACAT1 and TNF-α, IL-1β, MCP1, IL-10 was analyzed by simple linear regression. Result: Compared with normal aortic valve, senile degenerative active valve was thickening, calcified nodules existed, the valves lost the original structure because of the the proliferation fibrous tissue disarranging, macrophages and oth er inflammatory cells, foam cells, angiogenesis were observed around of calcified nodules. Macrophages were not detected in normal aortic valves, while were detected in DAVD valves. Compared with normal aortic valves, the expression of ACAT1 and ABCA1 increased in DAVD (P〈0.05), the expression of CD68 did not detected in nor mal aortic valves, while observed in DAVD. Linear regression analysis showed the expression of CD68 was posi- tively correlated with ACAT1 (r=-0.89, P〈0.05) and ABCA1 (r=-0.83, P〈0.05). Compared with nor-mal aortic valve, the expression of TNF-α,IL-1β and MCP1 increased in DAVD, and IL-10 decreased (P〈0.05). Linear regression analysis indicated the expression of CD68 was positively correlated with TNF-α (r= 0.87, P〈 0.05), IL-1β (r=0.92, P〈0.05) and MCP1 (r=0.87, P〈0.05), while negative correlated with IL-10 (r= -0.64, P〈0.05).
