摘 要:目的了解奉化市实施儿童免疫接种信息化系统管理后儿童常规免疫接种率水平,发现和掌握常规免疫管理工作中的薄弱环节及影响因素。方法从浙江省免疫规划信息系统中选取2013年奉化市1-7周岁(2006年1月1日-2012年12月31日出生)在册儿童的预防接种个案资料,分别对各年龄组儿童的免疫规划疫苗单苗单剂次接种率及含麻疹成分疫苗(MCV)首针及时接种率进行分析。结果2013年度全市在册登记的1~7周岁儿童共36139名,流动儿童占47.80%。本地户籍儿童免疫规划疫苗单苗单剂次接种率大多在95.00%以上,流动儿童在56.18%~100.00%之间。2~7岁组流动儿童第2剂次A群流脑多糖疫苗(MPV-A2)和第4剂次百白破疫苗(DTP4)接种率低于本地户籍儿童,大年龄组(6~7岁)尤为明显。7岁组儿童第2剂次A+C群流脑疫苗(MPV—AC2)接种率均偏低,流动儿童MPV.AC2接种率仅57.63%。在MCV首针接种及时性方面,适龄儿童第1剂次MCV及时接种率从2011年以前(3—7岁儿童)的75.92%上升至2011年后(〈3岁儿童)的92.46%,但各年龄段流动儿童的第1剂次MCV及时接种率仍显著低于本地户籍儿童。结论奉化市儿童常规免疫疫苗接种率维持在较高水平,但大年龄组的加强免疫(复种)剂次的接种率相对偏低,且流动儿童明显低于本地儿童。Objective To evaluate the routine immunization coverage of children in Fenghua and explore the relative determinants after using the Immunization Information System (IIS), so as to improve the management strategy. Methods Immunization records of children aged 1-7 years (born from January 1, 2006 to December 31, 2012) were collected from Zhejiang provincial IIS. Coverage rates for each vaccine (dose) recommended by National Immunization Program (NIP) and timeliness of the first dose measles containing vaccine (MCV) were analyzed by age groups. Results A total of 36 139 children aged 1-7 years were enrolled, 47.80% of which were migrants. Coverage rates of the vaccines (doses) for resident children mostly exceeded 95.00%, while those for migrants were between 56.18% and 100.00%. Coverage rates of the 2nd dose of group A meningococcal polysaccharide vaccine (MPV-A2) and the 4th dose of diphtheria pertussis tetanus vaccine (DTP4) for migrant children aged 2-7 years were significantly lower than resident children, especially for the children aged 6-7 years. Coverage of the 2nd dose of group A and C meningococcal polysaccharide vaccine (MPV-AC2) for the group of 7 years old was low, and the vaccination rate of migrant children was 57.63%. The timeliness vaccination rate of MCV1 raised from 75.92% before 2011 (3-7yearsold) to 92.46% after 2011 (〈3yearsold), but it was lower for migrant children compared with resident children in every age group. Conclusions The routine immunization coverage of children in Fenghua maintains a high level, but coverage of booster doses which scheduled to be given at an older age is relatively low, and migrant children was lower than resident children.
