临床药师参与病房抗感染治疗会诊分析

摘　　要：目的:总结并分析抗感染药物专业临床药师参与临床科室会诊的具体情况,以便为临床药师更好地参与临床实践提供参考。方法:对抗感染药物专业临床药师参与会诊的100例病例按感染部位、病原菌检出情况、会诊原因、会诊意见及采纳情况、患者预后等情况进行分析。结果:100例会诊病历中,感染部位依次为肺部(47%),颅内(14%);46例接受病原学检查的患者,检出53株病原菌,鲍曼不动杆菌检出率最高占21%;临床药师给出的会诊建议中44%为更换抗菌药物,15%为特殊抗菌药物应用会诊,13%为患者制定初始抗感染治疗方案;会诊意见被采纳率为98%,有2例未被采纳;100例会诊病例中治愈占31%,好转占50%,其余为患者死亡、无效及自请出院。结论:抗感染药物专业临床药师已经被越来越多的临床科室所接纳,并在参与感染性疾病的治疗中发挥着重要作用。Objective： To summarize and analyze specific condition of clinical consultation with Anti- infection clinical pharmacist participating, so as to provide reference for clinical pharmacists to better participate in clinical practice. Methods： A total of 100 cases of Anti-infection clinical pharmacists participating in consultation were summarized and analyzed statistically according to infection area, reason for consultation, consuhati onopinions and adoption, the prognosis of the patients. Results： In the 100 cases, lung infection was up to 47%, followed by intracranial infection, accounted for 14% ; 46 patients accepted etiological examination and then found 53 pathogenic bacteria, detection rate of Acinetobacter baumannii was up to 21% ; in the given suggestions for the consultation, 44% was pro- posed to replace antimicrobial drugs, 15% was the special use of antimicrobial drugs application con- sultation, 13% was participating in making initial anti-infection treatment for patients. 98% of consul- tation opinions were accepted by clinical doctors with 2 case denied. The results of 100 cases consulta- tion showed that the cure rate was 31%, the improvement rate was 50%, the others were death, inef- fective and discharged from hospital. Conclusions： The anti-infection clinical pharmacists are increas- ingly important in the infection treatment, and are accepted by more and more anti-infection clinical departments.
