磁共振成像对椎管内皮样或表皮样囊肿破裂的诊断价值

摘　　要：目的总结分析椎管内皮样囊肿或表皮样囊肿破裂的高场强磁共振成像(MRI)影像特征,旨在提高其诊断准确性。方法回顾性总结经手术及病理证实的9例椎管内皮样囊肿或表皮样囊肿破裂的高场强MRI资料并结合文献分析其影像学特征。结果 9例病变具有特征性表现:可见破裂的脂滴散布于蛛网膜下腔、脊髓中央管及脑室内,脂滴均呈短T_1、长T_2信号,黑水像呈低信号,扩散加权成像上呈低信号,增强扫描均未见强化,脂肪抑制序列短T_1信号均被抑制。其中1例脑室内可见脂肪-脑脊液平面及幕上脑室积水。结论 椎管内皮样或表皮样囊肿破裂具有特征性MRI影像学表现,磁共振T_1WI、脂肪抑制成像对该类病变的诊断及鉴别诊断具有重要价值,可实现术前正确诊断。Objective To study the characteristics of ruptured dermoid cyst or epidermoid cyst in intraspinal canal on the MRJ,in order to improve diagnosis accuracy.Methods The high field intensity MRI data of 9 cases with ruptured dermoid cyst or epidermoid cyst in intraspinal canal were summarized and analyzed on the basis of studying literatures,all of the cases were confirmed by pathology.Results The characteristics of this lesion by MRJ were showed as follows：lipid droplets could be revealed within subarachnoid space,central spinal canal and ventricle.Lipid droplets were showed as high signal on T_1WI and T_2WI,low signal on dark-fluid and DWI,with no enhancement.Fat-cerebrospinal fluid level within ventricles and supratentorial ventricles water could be seen in 1 case.Conclusion There are characteristic imaging features of ruptured dermoid cyst or epidermoid cyst,T,WI,fat-saturated sequences are of great value for diagnosis.
