摘　　要：目的探讨利用能谱CT低剂量对比剂肺动脉成像诊断肺动脉栓塞及肺梗死的价值。方法对我院2014年1~4月间怀疑肺动脉栓塞的30例患者,行能谱CT低剂量对比剂肺动脉成像,可获得常规CT肺血管造影图像、单能量图像及碘基物质图像,观察肺动脉内有无栓子,记录栓子数、栓子部位以及栓塞程度,分析碘基物质图的影像学表现,记录肺内灌注减低区的位置及数目。结果明确诊断肺动脉栓塞的患者12例,共检出36处肺动脉及其分支栓子,左、右肺动脉主干8处,肺叶动脉16处,肺段动脉12处,共测得肺内灌注减低区16处。结论能谱CT低剂量对比剂肺动脉成像不但能够降低对比剂用量,最大限度减少对比剂的不良反应的发生,而且利用最佳单能量图和碘基物质图及定量测量等工具,能够更准确、安全的诊断肺动脉栓塞及肺梗死。Objective To investigate the value of energy spectrum using low-dose CT imaging contrast agents in the diagnosis of pulmonary embolism and pulmonary infarction.Methods From January to March 2014,30 patients with suspected pulmonary embolism were subjected to dual energy spectral CT pulmonary artery angiography with low dose contrast medium in our hospital.By this technique,we obtained conventional CT pulmonary angiography images,single-energy image and the image of iodine-based substances,observed whether or not the pulmonary artery had emboli,recorded the number of emboli,emboli site and extent of thrombosis,analyzed imaging iodine-based substances map,recorded the location and the number of the reduced perfusion area in lung.Results The pulmonary embolism was definitely diagnosed 12 patients.A total of 36 emboli were found in the pulmonary artery and its branches,of which 8 were found in left and right pulmonary artery,16 in lung artery,12 in pulmonary artery segment.In all,16 reduced perfusion areas in lung were measured.Conclusion Spectrum of low-dose CT pulmonary angiography with contrast agent can not only reduce the amount of contrast agent to minimize the occurrence of adverse reactions to contrast agents,but take the advantages of the best single energy diagram and iodine-based substances,such as maps and quantitative measurement tools,to make more accurately and safely the diagnosis of pulmonary embolism and pulmonary infarction as well.
