弹性棒动态内固定系统在中青年单节段腰椎间盘突出症中的临床应用

摘　　要：目的 探讨腰椎后路弹性棒动态内固定系统治疗中青年单节段腰椎间盘突出症的临床疗效.方法 回顾性分析28例中青年单节段腰椎间盘突出症患者的病历资料,手术前后采用疼痛视觉模拟评分（VAS）及Oswestry功能障碍指数（ODI）进行比较评价.并动态观察术前术后病变上位节段椎间隙高度、手术节段活动度和腰椎总活动度变化情况.结果 末次随访时所有患者腰腿痛症状均有明显改善.术前VAS为6.51±1.88及ODI为（54.32±21.08）％,末次随访时VAS为1.38±0.36,ODI为（4.01±2.83）％,差异有统计学意义（P＜0.05）.随访影像学资料显示：术后复查患者手术节段ROM（关节活动度）减小,末次随访与术前比较差异有统计学意义（P＜0.05）,但末次随访时已恢复至术前近75％.末次随访时腰椎总活动度及病变上位节段椎间隙高度与术前比较差异未见统计学意义（P＞0.05）.结论 弹性棒动态内固定系统治疗中青年腰椎间盘突出近期临床效果满意,能保留手术节段的活动功能,减少或者减缓邻近节段退变.Objective To investigate the clinical efficacy of lumbar dynamic elastic rods,which is a kind of internal fixation system,on single segment of lumbar disc herniation in young people.Methods The clinical data of 28 cases of single segment of lumbar disc herniation in young patients were retrospectively analyzed.Before and after surgery,a visual analog pain score （visual analogue scale,VAS） and Oswestry Disability Index （oswestry disability index,ODI） were compared and evaluated.And the preoperative and postoperative lesions upper segment intervertebral height,surgical lumbar segmental mobility and overall activity changes were dynamicly observed.Results At last follow,all patients showed significant improvement of symptoms in patients with low back pain.Preoperative VAS was 6.51 ± 1.88 and ODI was（54.32 ± 21.08）% ; the last follow-up VAS was 1.38 ± 0.36,ODI was （4.01 ± 2.83）%,the differences were statistically significant （P 〈 0.05）.Follow-up imaging data showed：review of patients after surgery segment ROM（range of motion） was reduced,before the end of the follow-up of patients was statistically significant difference （P 〈 0.05）,but had been restored at the end of the follow-up to preoperative nearly 75%.There was no significant difference in the total lesion activity and upper lumbar segments intervertebral space height between the last follow-up and before operation （P 〉 0.05）.Conclusions The clinical effect of dynamic elastic rod fixation system on lumbar disc young term clinical results is satisfactory,it can retain the active function of surgery segments,and reduce or mitigate the adjacent segment degeneration.
