摘  要:目的 初步探讨人脐血干细胞(HUCBSC)对缺血性心肌病(ICM)患者心功能变化的影响.方法 选取湖北医药学院附属太和医院心内科2010年4月至2013年12月期间确诊缺血性心肌病患者29例,根据具体治疗情况分为对照组(14例)、外周静脉移植组(8例)和经冠状动脉移植组(7例).对照组给予规范的抗心衰药物治疗;细胞移植的两组在药物治疗基础上,分别经外周静脉和冠状动脉移植HUCBSC.分别记录三组在0月、1个月、3个月、6个月这4个时间点上体内脑钠肽(BNP)、左心室射血分数(LVEF)、6 min步行试验等观察指标的变化.结果 对照组、外周静脉移植组和经冠状动脉移植组间基线资料未见统计学差异.细胞移植组与对照组相比,在1个月后BNP开始下降,在3个月和6个月两个时间点上BNP下降更为明显,差异有统计学意义(P<0.05);而外周静脉移植组和冠状动脉移植组两组比较未见统计学差异(P.0.05).LVEF、6 rain步行试验等观察指标三组间未见统计学差异(P>0.05).结论 经外周静脉和冠脉移植HUCBSC治疗ICM能改善心功能,是一种安全有效的治疗方法.Objective To explore the effect of human cord blood stem cells transplantation therapy in pa- tients with ischemic cardiomyopathy by the changes of heart function. Methods 29 ICM patients from Hubei Medi- cal College Affiliated Taihe Hospital Department of Cardiology during 2010 April to 2013 December were divided in- to control group (14 cases), peripheral meridians transplantation group (8 cases) and intracoronary transplantation group (7 cases): the control group was given standard anti heart failure drug treatment, cell transplantation groups based on drug therapy, respectively by the peripheral veins and coronary artery transplantation HUCBSC. Changes were recorded, BNP, LVEF, 6-minute walk test as observation index. Results There was no statistically significant difference between the control group and the transplantation group with basic materials. Compared with the control group, one month later BNP in Cell transplantation group begin to decline, after six month BNP decreased more sig- nificantly, the results have statistical difference (P〈0.05). Between the peripheral vein transplantation group and the coronary artery transplantation group there was no differences (P〉0.05). There were no significance changes with 6- minute walk test and LVEF. Conclusion Through the peripheral vein and coronary transplantation of HUCBSC in treatment of ICM can improve cardiac function, and is a safe and effective treatment method.
