血清胱抑素C在早期诊断大鼠模拟高强度军事训练负荷累积肾损伤中的作用

摘  要:目的 探讨在大鼠模拟高强度军事训练致负荷累积肾损伤模型中,血清胱抑素C(cystatin C,Cys C)早期诊断的效能.方法 雄性4周龄SD大鼠40只构建高强度负荷累积肾损伤军事训练模型,按数字表法随机分为训练组(training group,T组)和对照组(control group,C组),每组20只大鼠.每2周处死T组及C组各5只,检测血清Cys C、血清肌酐(serum creatinine,SCr)、尿素氮(blood ureanitrogen,BUN)、内生肌酐清除率(creatinine clearance rate,CCr)、肾小管半定量评分.结果 大鼠训练8周过程中,血清Cys C在训练第4周起与SCr、BUN高于同期对照组(P<0.05),如第6周时,T组Cys C、Scr、BUN分别为(0.62±0.07) mg/L、(73.99±5.21) μmol/L、(8.36 ±0.73) mmol/L,C组Cys C、SCr、BUN分别为(0.48±0.09) mg/L、(51.06 ±8.72) μmol/L、(6.56±0.54) mmol/L.Cys C与BUN、SCr、最大摄氧量(VO2max)、同期肾小管积分正相关.受试者工作曲线(ROC)分析显示各点Cys C的曲线下面积(AUC)高于SCr、BUN.结论 血清Cys C对高强度军训致负荷累积肾损伤的诊断效能优于SCr、BUN,其升高与训练强度及肾病理损伤相关.Objective To investigate the value of serum Cystatin C (Cys C) in early diagnosis of chronic accumulative kidney lesion induced by simulated high-intensity military traiuing.Methods Forty male SD rats,4 weeks old,were used to establish the animal model of chronic accumulative kidney lesion induced by high intensity military training.During the training sessions,5 rats from the training group (or group T) and the control group (or group C) were sacrificed every 2 weeks to detect serum concentrations of Cys C,serum creatinine (SCr),blood urea nitrogen (BUN),creatinine clearance rate (CCr),and renal tubular semiquantitative integrals.Results During 8 weeks of training,levels of serum Cys C,SCr and Bun were higher at week 4,as compared with those of the control group(P 〈0.05).The level of Cys C was positively correlated with those of SCr,BUN,VO2max and renal tubular integrals.ROC analysis showed that AUC of the Cys C was higher than those of SCr and Bun at all the time points.Conclusions Serum Cys C in the diagnosis of chronic accumulative kidney lesion induced by simulated high-intensity military training was better than SCr and Bun.Elevated level of Cys C was closely correlated with high-intensity training and renal pathological lesion.
