蠲痹历节清方治疗急性期湿热蕴结型痛风性关节炎31例  

Clinical efficacy of Juanbilijie prescription in treatment of acute gouty arthritis of dampness- heat accumulation type: a report of 31 cases

摘　　要：目的:观察蠲痹历节清方治疗急性期湿热蕴结型痛风性关节炎的临床疗效。方法:将62例急性期湿热蕴结型痛风性关节炎患者随机分为两组各31例。对照组采用西药治疗,治疗组采用蠲痹历节清方治疗。对比观察两组治疗后的疗效、血尿酸和血沉水平的变化,临床症状、体征改善情况、不良反应和复发率等。结果:总有效率治疗组为90.3%,对照组为74.2%,组间比较,差异有统计学意义(P<0.05);两组血尿酸、血沉治疗前后组内比较及治疗后组间比较,差异均有统计学意义(P<0.05);两组症状、体征及高尿酸血症消失时间比较,差异均有统计学意义(P<0.05);两组治疗后不良反应发生率和复发率比较,差异均有统计学意义(P<0.05)。结论:蠲痹历节清方治疗痛风性关节炎急性期疗效明显,且复发率低,不良反应少,值得临床推广。Objective： To observe the clinical efficacy of Juanbilijie prescription in the treatment of acute gouty arthritis of dampness- heat accumulation type. Methods： A total of 62 patients with acute gouty arthritis of dampness-heat accumulation type were randomly divided into control group（ n = 31） and treatment group（ n = 31）. The control group was treated with Western medicine,and the treatment group was treated with Juanbilijie prescription. The clinical efficacy,changes in blood uric acid and blood sedimentation,clinical symptoms,improvement in signs,adverse reactions,and recurrence rate of the two groups after treatment were observed and compared. Results： The overall response rates of the treatment group and control group were 90. 3% and 74. 2%,respectively,and the difference was significant（ P〈0. 05）. The blood uric acid and blood sedimentation of the two groups showed significant differences within each group before and after treatment,and also showed significant differences between the two groups after treatment（ P〈0. 05）. The symptoms,signs,and hyperuricemia disappearing time of the two groups showed significant differences（ P〈0. 05）. The incidence of adverse reactions and recurrence rate of the two groups after treatment showed significant differences（ P〈0. 05）. Conclusion： Juanbilijie prescription has good efficacy in the treatment of acute gouty arthritis with low recurrence rate and few adverse reactions,and it should be promoted clinically.
