PPARγ及中药清胰汤对急性胰腺炎相关性肺损伤大鼠肺组织的保护作用探讨  被引量:9

摘  要:目的:探讨过氧化物酶体增殖物激活受体γ(PPARγ)及中药清胰汤对急性胰腺炎相关性肺损伤(APALI)的干预作用。方法:选用健康成年雄性Wistar大鼠60只,随机分为6组:假手术组、模型组、PPARγ配体罗格列酮(ROSI)处理组、PPARγ拮抗剂GW9662处理组、中药清胰汤处理组、PPARγ拮抗剂GW9662+中药清胰汤处理组,每组10只。采用胆胰管逆行注入1.5%的脱氧胆酸钠(0.1m L/100g)建立APALI模型,造模成功后给予药物干预,于给药6h后处死动物,测定动脉血气PCO2、PO2含量,观察肺组织病理学改变。结果:(1)模型组与假手术组相比,PCO2明显升高,PO2明显下降(P<0.05),肺组织出现充血、水肿、中性粒细胞浸润及灶性肺不张等改变。(2)PPARγ配体ROSI组、中药清胰汤处理组与模型组相比,PCO2明显下降,PO2明显回升(P<0.05),肺组织病理改变明显减轻。(3)PPARγ拮抗剂GW9662处理组与模型组相比,PO2、PCd含量均无显著性差异(P>0.05),肺组织病理改变亦无明显变化。(4)PPARγ拮抗剂GW9662+中药清胰汤处理组与中药清胰汤处理组相比,PCO2有所升高,PO2有所下降(P<0.05),肺组织病理改变加重。结论:(1)PPARγ配体激活PPARγ后,可使APALI模型大鼠精神活动和呼吸情况明显改善,肺组织病理损伤明显减轻,且上述效应可被PPARγ拮抗剂所逆转,表明PPARγ及其配体对APALI具有保护作用。(2)清胰汤是治疗APALI的有效方剂,其作用机制可能是通过直接或间接激活PPARγ,起到配体激动剂的作用。Objective: To investigate the intervention effects of peroxisome proliferator- activated receptor gamma( PPARγ) and Qingyi decoction( traditional Chinese medicine) on acute pancreatitis- associated lung injury( APALI). Methods: Sixty adult healthy male Wistar rats were equally and randomly divided into six groups: sham-operation group,model group,rosiglitazone( a ligand of PPARγ,ROSI) treatment group,GW9662( an antagonist of PPARγ) treatment group,Qingyi decoction treatment group,and GW9662 + Qingyi decoction treatment group( n = 10 for each). An APALI model was induced by retrograde injection of 1. 5% sodium deoxycholate( 0. 1 m L /100g) into the biliopancreatic duct in rats. After the model was successfully established,the rats were given medical intervention and were sacrificed at 6 h after medical intervention. The partial pressure of carbon dioxide( PCO2) and partial pressure of oxygen( PO2) in arterial blood were determined,and the pathological changes in lung tissue were observed. Results:( 1) Compared with the sham- operation group,the model group had significantly increased PCO2 and decreased PO2( P〈0. 05),and had lung congestion,edema,neutrophil infiltration,focal atelectasis,and other changes.( 2) Compared with the model group,the ROSI treatment group and Qingyi decoction treatment group had significantly decreased PCO2 and significantly increased PO2( P〈0. 05),and the pathological changes in lung tissue were significantly reduced.( 3) There were no significant differences in PO2 and PCO2and the pathological changes in lung tissue between the GW9662 treatment group and the model group( P〉0. 05).( 4) Compared with the Qingyi decoction treatment group,the GW9662 + Qingyi decoction treatment group had significantly increased PCO2 and decreased PO2( P〈0. 05),and the pathological changes in lung tissue were increased.
