针刀微创治疗慢性膝关节周围炎的临床研究

摘　　要：目的观察针刀微创治疗慢性膝关节周围炎的临床疗效。方法选择疼痛门诊确诊为慢性膝关节周围炎患者200例,半随机分为观察组和对照组,每组100例。观察组神经阻滞后进行针刀微创松解治疗,对照组进行单纯神经阻滞治疗。比较两组治疗前及治疗后1 w、2 w、4 w后不同时点静息疼痛VAS评分(rest visual analogue scale,RVAS)和被动活动时VAS评分(passive visual analogue scale,PVAS),比较两组综合疗效。结果两组治疗后RVAS评分和PVAS评分均呈下降趋势,观察组总体评分均低于对照组,差异具有统计学意义(P<0.05)。与对照组相比,观察组治疗后2 w、4 w疼痛RVAS评分和PVAS评分均明显下降(P<0.05),观察组疗效明显优于对照组,差异具有统计学意义(P<0.01)。结论针刀微创治疗慢性膝关节周围炎疗效显著,远期疗效优于单纯神经阻滞治疗。Objective To observe the clinical therapeutic effect of minimally invasive needle knife treatment of chronic knee periarthritis. Methods By semi-randomized method,200 cases of chronic knee periarthritis were divided into observation group and control group,100 cases in each group. The observation group received needle knife minimally invasive lysis treatment after nerve block,while the control group only received the treatment of nerve block. Before treatment and at 1 w,2 w and 4 w after treatment resting pain VAS score（ rest visual analogue scale,RVAS） and the passive activity VAS score（ passive visual analogue scale,PVAS） were compared between the two groups; and the comprehensive curative effect was compared between the two groups. Results After treatment,RVAS score and PVAS score in the two groups decreased,and the overall score of the observation group was lower than that of the control group,with significant difference（ P〈0. 05）. Compared with the control group,pain RVAS score and PVAS score at 2 w and 4 w in the observation group were significantly decreased after treatment（ P〈0. 05）. The therapeutic effect of observation group was better than that of the control group（ P〈0. 01）. Conclusion The minimally invasive needle knife treatment has a significant efficacy in the treatment of chronic knee periarthritis,and its long-term efficacy is superior to single nerve block treatment.
