摘　　要：目的:探讨丁苯酞对急性脑梗死患者血清脑源性神经营养因子(BDNF)水平和神经功能的影响。方法:60例急性脑梗死患者随机分为对照组(n=30)和治疗组(n=30),所有患者根据缺血性卒中诊治指南给予规定药物,治疗组加用丁苯酞。在治疗前以及治疗2周和6周时检测血清BDNF水平,采用NIHSS评定神经功能缺损程度。结果:治疗组治疗2周(P=0.045)和治疗6周时(P=0.034)NIHSS评分均显著性低于对照组。两组治疗后血清BDNF水平均显著性增高,治疗组血清BDNF水平在治疗2周时(P=0.015)和6周时(P=0.021)均显著高于对照组。血清BDNF水平与NIHSS(r=-0.705,P<0.001)呈显著性负相关。结论:丁苯酞治疗急性脑梗死能更有效地促进神经功能恢复,其机制可能与提高血清BDNF水平相关。Objective： To investigate the effects of Butylphthalide on the levels of serum brain-derived neu- rotrophie factor （BDNF） and neurological function in patients with acute cerebral infarction. Methods： Sixty patients with acute cerebral infarction were randomly divided into either a control group （n=30） or a treatment group （n=30）. All patients received the prescribed drugs according to the diagnosis and treatment the guidelines for ischemic stroke. The treatment group added Butylphthalide. Before and at two and six weeks of treatment, the serum BDNF levels were detected. The National Institutes of Health Stroke Scale（NIHSS） was used to evaluate the activities of daily living. Re- sults：At 2 （P=0.045） and 6 （P=0.034） weeks after treatment, the scores of NIHSS of the treatment group were significantly lower than those of the control group. After treatment, the serum BDNF levels were increased significantly in both groups. At 2 weeks after treatment, the serum BDNF levels（P=0.015） and at 6 weeks after treatment, those（P=0.021） in the treatment group were significantly higher than those in the control group. The serum BDNF levels were significantly negatively correlated with the scores of NIHSS（r=-0.705, P〈0.001）. No serious adverse reaction occurred in two groups. Conclusion： Butylphthalide for the treatment of acute cerebral infarction can more effectively promote the recovery of neurological function, and its mechanism may be associated with the increased serum BDNF levels.
