疣毒净系列联合激光治疗肛门和阴道尖锐湿疣的临床研究

摘　　要：【目的】观察中药疣毒净胶囊、疣毒净栓联合激光治疗尖锐湿疣(condyloma acuminatum,CA)的临床疗效。【方法】将符合肛门或阴道CA诊断的90例患者随机分为试验组和对照组各45例。均采用激光气化或切除肉眼可见的疣体及醋酸白试验阳性的皮损,术后常规给予抗炎药和1︰8 000稀释后的高锰酸钾溶液泡洗,皮疹结痂后停用。试验组加用中药疣毒净胶囊和疣毒净栓治疗2个月,术后第3个月后判定临床疗效。【结果】(1)依从性方面:试验组脱落2例,对照组脱落5例,共83例完成临床观察。(2)疗效方面:激光术后3个月,试验组疗效优于对照组(P<0.05)。(3)干预前疣体数与疗效的关系:对照组对单发或散在(≤4个)和多发疣体(>4个)的干预效果相当(P>0.05),而试验组对单发或散在疣体的干预效果优于多发疣体(P<0.01)。(4)在复发患者中,试验组的干预治疗能有效减少疣体的复发数目,与对照组比较差异有统计学意义(P<0.01)。【结论】疣毒净系列联合激光治疗肛门和阴道尖锐湿疣的临床疗效优于单纯激光治疗,且具有安全性好、使用方便、价格适中等优点,值得临床推广应用。Objective To observe the clinical efficacy of Youdujing capsules and suppository plus laser therapy for the treatment of anogenital condyloma acuminatum（CA）. Methods Ninety patients diagnosed as anogenital CA were randomly divided into experimental group and control group, 45 cases in each group. For all of the patients, laser therapy instrument was used to gasify or remove the visible wart and the lesion with acetic acid white test positive, and then conventional anti-inflammatory drugs were used to prevent infection and diluted potassium permanganate solution（at the concentration of 1：8000） was used to soak and clean the infected parts till the formation of a scab. Additionally, patients in the experimental group took Youdujing capsules orally（5 capsules, tid） after operation and were given anal or vaginal use of Youdujing suppository（one suppository,once every other night） one week after operation, and the medication lasted for two months. The clinical efficacy was evaluated 3 months after operation. Results（1） In the experimental group, 2 cases dropped out, and in control group, 5 cases dropped out. Finally, a total of 83 cases completed the trial.（2） The therapeutic effect of the experimental group was superior to that of the control group 3 months after operation（ P〈0.05）.（3） In the control group, the effect on single or scattered warts less than 4 was similar to that on multiple warts over 4（P〉0.05）. In the experimental group, the effect on single or scattered warts less than 4 was better than that on multiple warts over 4（P〈0.01）.（4） The number of recurrent warts in the experiment group was less than the control group, the difference being significant（ P〈0.01）. Conclusion Youdujing capsules and suppository plus laser therapy for the treatment of anogenital condyloma acuminatum are effective, safe,convenient and practical, showing better efficacy than laser therapy alone, being worth of recommendation in the clinical application.
