中青年国人正常左心室整体功能MSCT研究  

Evaluation of left ventricular global function in healthy young and middle-aged people with MSCT during coronary CTA

摘  要:目的探讨256层CT冠状动脉成像时正常中青年国人左心室(LV)整体功能正常值及其与性别、年龄的关系。方法410例LV整体功能分析指标包括舒张末期容积(EDV)、收缩末期容积(ESV)、每搏输出量(SV)、每分心输出量(CO)、心肌质量(MM)、射血分数(EF)、标准化舒张末期容积(EDVI)、标准化收缩末期容积(ESVI)、标准化每搏输出量(SVI)、心脏指数(CI)、心肌质量指数(MMI)正常值,观察性别间心功能参数差异、心功能与体重指数(bodymassindex,BMI)及年龄之间的相关性。结果LV整体功能参数,男性为:ESV47.10~55.33ml,EDV143.15—154.56ml,SV94.49—10.96ml,CO6162.59~6734.39ml/min,MM129.78~146.00g,EF64.17%-67.69%;女性为:ESV39.62~45.33ml/m2,EDV126.49~136.26ml/m2,SV85.43.92.35ml/m2,CI5590.52-6129.45ml·min-1·m-1。MM10.98~113.87g/m2,EF65.87%~69.00%。男性组LV整体功能参数值明显高于女性组,标准化后,仅MMI较高。BMI与左心室整体功能参数相关性tEDV、ESV、SV、CO、MM与BMI呈正相关(r值分别为0.293、0.424、0.335、0.199、0.482)。标准化后MMI与BMI呈负相关(r=0.276,P〈0.05),EDVI、ESVI、SVI、cI与BMI无相关性。年龄与LV整体功能参数:EDV、ESV与年龄呈负相关(r=-0.362,-0.436,P〈0.05)。结论256层CT可方便地获得正常中青年人LV整体功能MSCT参数,各参数与性别及年龄、BMI有一定关系。Objective To setup the normal range of left ventricular (LV) global function parameters in healthy young and middle-aged people, and to explore the relationship between the gender, age, body mass index(BMI) and LV global function parameters. All these cases did not detect any abnormalities about coronary and heart when having coronary artery CTA with 256 slice CT. Methods To the 410 normal cases, end-diastolic volume ( EDV ), end-systolic volume ( ESV ), stroke volume( SV), ejection fraction ( EF ), cardiac output ( CO ), myocardial mass ( MM ), end-diastolic volume index ( EDVI ), end-systolic volume index (ESVI) , stroke volume index (SVI) , cardiac output index (CI) , myocardial mass index (MMI) were measured. The difference of the parameters between the two genders, the relationship between the LV global function parameters and BMI,age were evaluated. Results The normal range of left ventricular (LV) global function are:male,ESV47.10~55.33ml,EDV143.15-154.56ml,SV94.49-10.96ml,CO6162.59~6734.39ml/min,MM129.78~146.00g,EF64.17%-67.69%;ESV39.62~45.33ml/m2,EDV126.49~136.26ml/m2,SV85.43.92.35ml/m2,CI5590.52-6129.45ml·min-1·m-1.MM10.98~113.87g/m2,EF65.87%~69.00.. EDV,ESV,SV,CO,MM in male group are significantly higher than those in female group( P 〈0.05). Wheras,for the standardized parameters,only MNI in male group is larger than that in female group ( P 〈 0.05 ). There is no correlation between BM1 and EF( P = 0. 194 ), while there are definitive correlation between BMI and EDV, ESV, SV, CO, MM ( P = 0. 002,0. 000,0. 000,0. 038,0. 000, r = O. 293, 0. 424,0. 335,0. 199,0. 482). After standardization, only MMI is correlated with BMI, ( P = 0.04, r = O. 276). Age is significantly correlated with ESV and EDV ( P = O. 017,0. 003, r = - 0. 362, - 0.436) before standardization. But, after standardization, there is no correlation between the age and all parameters. Conclusion It is convenience to get the normal rang
