经济人视角下医疗趋利行为及其制约机制

摘　　要：根据"经济人"假设理论,分析经济活动中行为主体客观存在的自利性特征,探究我国公立医疗机构及其医务人员医疗趋利行为表现的根源。发现市场经济条件下,医疗趋利行为的存在具有客观必然性,且因我国尚不完善的医疗服务市场机制而助长,并造成公立医疗机构公益性缺失和医患关系恶化。针对目前主要从道德约束和法律规范角度遏制医疗趋利行为的现状,提出应从财政补偿、公益性考评、利益制约、临床路径、薪酬制度等制约机制的构建上来根本遏制医疗趋利行为的建议。Based on the theory of ＂economic man＂ hypothesis, this article analyzed the characteristic of selfishness in economic activity and the primary reasons that public medical institutions and individuals behave interests-oriented behavior of medical treatment in China. It found that the interests-oriented behavior is an objective existence in the market economy, but it is intensified in China because of the imperfect medical service system, and has bad influences to public welfare and doctor-patient relationship. For restricting the interests-oriented behavior, aiming at the condition of mainly taking measures from the perspective of morality and law in China, this article put forward some suggestions from government compensation, welfare assessment, interest restriction, clinical pathway and salary system in terms of establishing the benefit restriction mechanism.
