摘  要:目的探讨对妊娠期尿碘低下的孕妇进行及早干预,从而预防新生儿先天性甲状腺功能减低症(CH)发生的实践意义。方法对孕妇进行尿碘及血清甲状腺功能指标测定,尿碘结果异常者指导其调整饮食及药物干预,并于新生儿出生后第三天采其足跟血测定TSH,以对干预效果进行评估。结果尿碘低下组孕妇FT3为2.84±0.39pg/m L,FT4为1.07±0.16ng/d L,TSH为1.85±1.63μIU/m L,新生儿TSH为2.37±2.09m U/L,其中>5m U/L的占9%;尿碘正常组孕妇FT3为2.90±0.37pg/m L,FT4为1.07±0.18ng/d L,TSH为1.75±1.74μIU/m L,新生儿TSH为2.27±1.98m U/L,其中>5m U/L的占10%。两组四项指标的差异均无统计学意义,t值分别为1.00、0.07、0.41、0.36,P>0.05。结论通过尿碘筛查可评估孕妇碘代谢状况,从而能够在产前对预防CH进行及早干预以降低该病的发生率并取得良好的效果,并因该技术的无创性也便于对孕妇碘代谢状况进行连续监测。Objective:To explore the practical values of preventing the incidence of congenital hypothyroidism(CH)in antenatal by early intervention to the pregnant women whoes low lever urinary iodine. Methods:Urinary iodine and Serum index of thyroid function were detected to pregnant women during antenatal. The results of urinary iodine were abnormal to be guided the adjustment diet and drug intervention. And on the third day after birth by the heel blood TSH to evaluate effectiveness of intervention. Results:Low urinary iodine group of pregnant women FT3 was 2.84±0.39 pg/m L,FT4 was 1.07±0.16 ng/d L,TSH was 1.85±1.63 μIU/m L,neonatal TSH was 2.37±2.09 m U/L,of whom 9% 〉5 m U/L;Urinary iodine group normal pregnant women FT3 was 2.90±0.37 pg/m L,FT4 was 1.07±0.18 ng/d L,TSH was 1.75±1.74μIU/m L,neonatal TSH was 2.27±1.98 m U/L,of whom 10%〉 5 m U/L. There was no statistically significant difference of the indicators in both groups,t value were 1.00,0.07,0.41,1.00 respectively,P〉0.05. Conclusion:Through the urinary iodine screening can assess iodine metabolism of pregnant women. So that they can to prevent the incidence of CH in prenatal by early intervention and achieve good results. And because of its non-invasive also facilitate for continuous monitoring of pregnant women iodine metabolism.
