摘  要:目的探讨本地区0~16岁儿童胰岛素样生长因子-1(IGF-1)的变化并建立其正常参考范围。方法 2009年至2014年共筛选276例0~16岁正常儿童(男156例,女120例),采用化学发光法检测检测其血清的IGF-1浓度,分析血清IGF-1随年龄增长的变化趋势并建立婴儿期、幼儿期、学龄前期、学龄初期、学龄中期、青春期6个时期的正常参考范围。结果儿童血清IGF-1水平随着年龄的增长而升高,男、女孩分别于14岁和13岁时出现高峰;高峰值后,IGF-1水平随年龄增长缓慢下降或为平台期;276例儿童按婴儿期、幼儿期、学龄前期、学龄初期、学龄中期、青春期的结果依次为中位数40.5,79.0,117,184,284,342;参考范围分别为30.4-62.5;29.7-156;63.7-201;123-322;156-508;211-532,各年龄组间IGF-1浓度差异有统计学意义(P〈0.05)。结论建立了本地区儿童血清IGF-1的正常参考范围,对于生长监测、临床诊断及GH治疗后随访有重要参考价值。Objective:To investigate the changes of serum insulin-like growth factor-1(IGF-1)in normal children aged 0to 16,and to establishment the reference ranges. Methods:Serum IGF-1 of 276 cases of normal children aged 0 to 16(156boys,120girls)were determined by chemiluminescence from July 2009 to Feb 2014.Comprehensived analysis the serum IGF-1 along with the age growth trends and established the infancy,the early infancy,preschool and school-age,middle school age and adolescence,six stages the normal reference range. Results:The serum IGF-1 level increased gradually as the growth of the age,the peak of the boy and girl was age 14 and 13 respectively. IGF-1 slowly decline or stayed as peak value in the plateau afther peak. The result of the median and reference range in 276 cases of children in infancy,early infancy,preschool,school age,the middle school age and adolescence was following:40.5、(30.4-62.5);79.0、(29.7-156);117、(63.7-201);184、(123-322);284、(156-508);342、(211-532). Serum IGF-1 concentration differenced between each age group was statistically significant(P 0.05). Conclusion:The normal reference range of serum IGF-1 in our district was established,and it′s important for growth monitoring,clinical diagnosis and follow-up after GH treatment.
