摘 要:目的:分析乌司他丁(UTI)在脓毒症所致大鼠急性肾损伤(AKI)发病机制中的作用。方法:选用42例雄性健康SD大鼠依随机原则分为:对照组,予以静脉接种1ml PBS;脓毒症组,通过接种LPS构建AKI模型;处理组,在构建AKI动物模型基础上再予以UTI干预。每组14例大鼠。干预12h后,应用ELISA法检测三组大鼠血清中白细胞介素(IL)-6、IL-8、IL-18及TNF-α等各细胞因子水平的差异;通过测定血清及尿液中肌酐水平与尿量,计算大鼠内生肌酐清除率(Ccr)的差异;应用免疫组织化学法检测肾脏组织中核因子κB(NF-κB)表达水平的差异。结果:脓毒症组大鼠的IL-6、IL-8、IL-18及TNF-α细胞因子水平最高(P<0.05),而对照组与处理组大鼠的各细胞因子无显著差异(P>0.05)。脓毒症组大鼠的血清肌酐(Scr)水平最高,而尿肌酐(Ucr)与Ccr水平最低(P<0.05),而对照组与处理组大鼠则无显著差异(P>0.05)。肾脏组织中NF-κB表达水平为:脓毒症组>处理组>对照组(P<0.05)。结论:UTI对大鼠脓毒症诱导发生AKI的肾脏损伤具有保护作用。Objective: To analyze the effect of ulinastatin(UTI) in acute kidney injury(AKI) induced by sepsis in rats.Methods:The 42 male SD rats were randomly divided into three groups,control group(1 ml phosphate buffer solution injected via caudal vein),sepsis group(LPS injected via caudal vein to establish AKI model) and interfere group(UTI injected via caudal vein based on AKI model).Each group had 14 male SD rats.The cytokines of IL-6,IL-8,IL-18 and TNF-α were detected by ELISA and compared among three groups after 12 hours interfere.The levels of serum creatinine(Scr),urine creatinine and urine volume in three groups were detected and the endogenous creatinine clearance(Ccr) in three groups were calculated by above indexes.The concentrations of NF-κB in kidney tissue in three groups were detected by immunohistochemical method.Results:The levels of the cytokines of IL-6,IL-8,IL-18 and TNF-α in sepsis group were highest(P〈0.05),and the levels of above cytokines were no different between control group and interfere group(P〉0.05).The levels of Scr in sepsis group were highest,but Ucr and Ccr in sepsis group were lowest(P〉0.05),and the levels of Scr,Ucr and Ccr were no different between control group and interfere group(P〈0.05).The sequence of NF-κB concentrations in kidney tissue in three groups were sepsis group〉interfere group〉control group(P〈0.05).Conclusion:UTI may protect the AKI induced by sepsis in rats.
