腹腔镜胃转流术治疗2型糖尿病的临床体会(附36例报告)

摘　　要：目的：探讨应用腹腔镜胃转流术治疗2型糖尿病及其病情控制方面的临床效果。方法：分析2010年4月至2011年4月为36例2型糖尿病患者行腹腔镜胃转流术的临床资料,探讨糖尿病治疗及病情控制的效果,总结术后并发症情况及远期治疗效果。结果：36例均成功完成腹腔镜手术,术后无严重并发症发生。手术时间115~203 min,平均（136.4±21.3）min;术后可早期下地活动,术后平均住院（14.0±3.2）d,出院后约半个月可恢复正常工作生活。出院后对患者进行长期随访,患者均恢复良好,远期血压、血糖及血脂控制较好,糖尿病伴肥胖患者减重效果较好,且差异均有统计学意义（P〈0.05）。结论：腹腔镜胃转流术治疗糖尿病、控制各种并发症效果较好,尤其合并肥胖的患者,减重效果更佳,临床应用前景较可观。Objective： To investigate the clinic effects of laparoscopic Roux-en-Y gastric bypass（ LRYGB） in treating and controlling type 2 diabetes mellitus. Methods： The clinical data of 36 patients of type 2 diabetes mellitus after LRYGB from the fourth affiliated hospital of harbin medical university during Apr. 2010 and Apr. 2011 were analyzed,in order to investigate the effect of treating and controlling type 2 diabetes mellitus and summarize their postoperative complications and long-term efficacy. Results： The procedures were completed with laparoscopy successfully in all the cases. There were not serious postoperative complications. The time of operation ranged from 115 to 203 min with a mean of（ 136. 4 ± 21. 3） min. Patients were discharged from the hospital with a mean of（ 14. 0 ±3. 2） d after the surgery. They could make postoperative ambulation early and resume normal work and life about half a month after discharge. They were followed up for a long time after discharge,all patients recovered well. Their blood pressure,blood glucose and blood lipid were controlled well for a long time. There was a good effect of losing weight for obese patients,and the differences had statistical significance（ P〈0. 05）. Conclusions： LRYGB is an obvious treatment for type 2 diabetes mellitus and various complications. It has an especial effect of losing weight for obese patients,and it has a considerable clinical application prospect.
