快速康复外科理念应用于肝胆管结石病患者围手术期的效果观察

摘　　要：目的探讨快速康复外科(fast track surgery,FTS)在肝胆管结石病患者围手术期的应用效果。方法我院2009年1月至2014年4月120例择期开腹手术的肝胆管结石病患者,按随机数字表法分为FTS组和传统治疗组各60例。FTS组采用快速康复理念下围手术期处理方法,而对照组采用传统方法进行围手术期处理。比较两组患者术后下床活动时间、进食时间、首次排便时间、住院时间、住院费用和并发症发生率。结果与传统治疗组比较,FTS组术后下床活动、进食、首次排便时间提前,住院时间缩短,住院总费用减少,差异均有统计学意义(P<0.05)。FTS组切口感染发生率低于传统治疗组(P<0.05),其余并发症差异无统计学意义(P>0.05)。结论肝胆管结石病患者围手术期应用FTS理念能加快患者康复,且安全有效,值得临床推广应用。Objective To investigate the efficacy of using fast-track surgery（ FTS） in intrahepatic lithiasis during perioperative period. Methods The clinical data of 120 patients with intrahepatic lithiasis undergoing surgical treatment from January 2009 to April2014 were randomly divided into FTS and control groups,60 in each group. Patients in the FTS group received the new perioperative care procedure,namely the fast-track methodology,while those in the control group underwent the conventional perioperative treatment.The postoperative time for bowel flatus,first defecation,hospital stay,hospitalization cost,and postoperative complication were compared between the two groups. Results Compare to the control group,the time of ambulation,food intake and the first defecation in the FTS group was advanced,and the hospital stay and the total cost of hospitalization was decreased. The differences were all statistically significant（ P〈0. 05）. The incidence of wound infection in the FTS group was significantly lower than that in the control group although no difference in other complications was found between the two groups. Conclusion The application of FTS in perioperative management of intrahepatic lithiasis is safe and effective and can accelerate the recovery of patients. It is worthy of clinical application.
