农村失能老年人护理需求调查

摘　　要：目的了解农村失能老年人护理需求现状及影响因素,为建立适合农村地区失能老年人的社区护理服务提供理论依据。方法采用分层整群抽样方法,应用自行设计的一般资料调查表和护理需求量表对开封市398名农村失能老年人进行入户调查。结果农村失能老年人护理需求普遍较高,其中在疾病预防方面,定期提供健康体检得分最高(4.67±0.57)分;在健康促进方面,建立健康咨询热线电话(4.67±0.57)分需求量最大。子女有无外出打工、配偶健康状况、失能程度、就医地点在单因素分析中差异有统计学意义(P<0.05,P<0.01);逐步回归分析显示配偶健康状况、失能程度、就医地点是影响农村失能老年人护理需求的主要因素(P<0.05,P<0.01)。结论农村失能老年人的护理需求处于较高水平,且影响因素较多。在建立发展农村社区护理服务时,应考虑失能老年人群体的特殊性,制定相应的服务措施,以满足其护理需求。Objective This study aimed to understand the nursing needs and influencing factors among the disabled elderly in rural area and to provide theoretical basis for the establishment of community care services to the disabled elderly in rural area. Methods By using stratified cluster sampling method, 398 disabled old people in rural area of Kaifeng were face-to-face interviewed with self-designed general information questionnaire and nursing needs questionnaire. Results The nursing needs of the disabled in rural area are generally higher. Providing regular checkups earn the highest scores（4.67±0.57） in terms of disease prevention, and the demand for establishing health consultation hotline（4.67±0.57） is the highest for health promotion. Children working outside or not, the spouse health status, degree of disability and the place for medical care have showed statistically significant effect by univariate analysis（P〈0.05）. And the spouse health status, degree of disability and the place for medical care are the main factors influencing the nursing needs of the rural disabled elderly in stepwise regression analysis（P〈0.01）. Conclusion Nursing needs of the disabled elderly in rural area is high and affected by many factors. In order to meet their nursing needs, the particularity of disabled elderly should be considered when providing the rural community nursing service.
