摘  要:2型糖尿病胰岛素抵抗的中医基本病机是在脾胃亏虚基础上产生痰浊、瘀血,脾胃亏虚为本,痰瘀互结为标。其与营养素通路关系密切,且发病过程与营养素通路激活机理一致,故改善营养素通路成为胰岛素抵抗等代谢性相关疾病新的治疗策略。针对目前糖尿病胰岛素抵抗中存在的指标选择不能体现病证结合、中医证候动物模型较少、缺乏大样本多中心临床随机对照研究等不足,提出今后中医研究应从临床和动物实验两个方面、动物-细胞-分子三个水平、宏观与微观两个层次入手,通过观察营养素通路的改变,深入研究2型糖尿病胰岛素抵抗的发病机制。The traditional Chinese medicine( TCM) pathological mechanism of type 2 diabetes is that phlegm turbidity and blood stasis generated on the basis of spleen and stomach deficiency. Spleen and stomach deficiency is the root cause,and phlegm and static blood binding together is the branch cause. It is closely related to nutrient pathway. And its pathogenesis process is consistent with the activation mechanism of nutrient pathway. Therefore,to improve nutrient pathway becomes a new therapeutic strategy for insulin resistance and other related diseases. The following limitations exist in recent insulin resistance studies: selection of indicators does not reflect the combination of disease and syndrome; animal models of TCM syndrome in insulin resistance are not enough; and there are few large samples,multi-center,randomly controlled studies in clinical research. To solve these problems,this paper proposes that the future study should be carried out from two respects( clinical and animal experiments),three levels( animals,cell,and molecules),and two perspectives( macro and micro). The pathogenesis of insulin resistance in type2 diabetes should be deeply studied through observing changes in nutrient pathway.
