增液通便方敷脐联合蜡袋灸治疗气阴两虚型2型糖尿病便秘患者30例临床观察

摘　　要：目的探讨增液通便方敷脐联合蜡袋灸治疗气阴两虚型2型糖尿病便秘患者的临床疗效。方法 90例气阴两虚型2型糖尿病便秘患者随机分为敷脐组、敷脐加灸疗组、对照组(六味安消胶囊)各30例,3组疗程均为2周。于治疗前、治疗后、停药3个月后(随访时)观察便秘症状积分、中医自我症状积分和生活质量评分,并判定临床疗效。结果与治疗前比较,敷脐组和敷脐加灸疗组治疗后、随访时便秘症状及中医自我症状积分降低,生活质量评分升高(P<0.01);与对照组同期比较,敷脐组和敷脐加灸疗组在治疗后、随访时便秘症状及中医自我症状积分均降低(P<0.05),敷脐加灸疗组生活质量评分明显升高(P<0.05)。3组治疗后临床疗效总有效率比较,差异无统计学意义(P>0.05),随访时敷脐组(71.43%)及敷脐加灸疗组(89.66%)临床疗效明显高于对照组(20.00%),且敷脐加灸疗组的总有效率明显高于敷脐组(P<0.01)。结论增液通便方敷脐治疗气阴两虚型2型糖尿病便秘的效果优于六味安消胶囊,且增液通便方敷脐联合蜡袋灸对生活质量的影响和对便秘效果的远期疗效优于单纯敷脐治疗。Objective To observe the effect of navel application of Zengye Tongbian Formula（ ZTF） combined with waxbag moxibustion（ WM） in the treatment of diabetic constipation with qi and yin deficiency. Methods Totally90 cases of diabetic constipation with qi and yin deficiency were randomly assigned to treatment group A（ navel application of ZTF）,treatment group B（ navel application of ZTF with WM） and control group（ Liuwei Anxiao Capsules）. The course of the treatment lasted for 2 weeks. The constipation symptom score,TCM symptom scores and quality of life（ QOL） were evaluated before treatment,two weeks after treatment and 3 months after the ending of treatment to determine the clinical efficacy. Results The constipation symptom scores and TCM symptom scores declined,while the quality of life improved significantly after treatment and during follow-ups in the treatment groups A and B（ P〈0. 01）. Compared with the control group,the constipation symptom scores and TCM symptom scores in the treatment groups A and B were significantly lower（ P〈0. 05）,and the QOL in group B was significantly higher（ P〈0. 05）. There was no significant difference in the total efficiency rates among the groups after treatments（ P〈0. 05）. During follow-ups,the efficiency rates in group A and B were 71. 43% and 89. 66%,respectively,which were significantly higher than the 20. 00% in the control group and indicated that group B had better results（ P〈0. 01）. Conclusion The clinical effect of navel application of ZTF was better than Liuwei Anxiao Capsules in the treatment of diabetic constipation with qi and yin deficiency. Moreover,the effect on QOL and long-term effect on constipation of navel application of ZTF with WM were superior to navel application alone.
