摘 要:目的观察通心络超微粉联合外周血间充质干细胞移植治疗糖尿病大鼠下肢缺血的疗效。方法采用高脂喂养联合低剂量链脲佐菌素(STZ)注射建立糖尿病大鼠模型,再结扎右下肢股动、静脉制造下肢缺血模型。100只造模成功大鼠随机分为模型组、通心络组、干细胞组、联合治疗组,每组20只,以同批次20只健康大鼠为正常组。通心络组和联合治疗组给予通心络超微粉0.4g/(kg·d)灌胃,连续治疗28天;干细胞组和联合治疗组于术后第3天在缺血下肢皮下肌层注射约0.4 ml细胞悬浊液(PB-MSCs1.0×106/ml),分8个位点,每个点注射0.05 ml;正常组、模型组和干细胞组大鼠以等体积的生理盐水灌胃,同点、同量肌肉注射无菌RPM1640培养液。分别于术前、术后、治疗7和28天行激光多普勒血流灌注成像检测计算双后肢血流差值,治疗28天后行下肢动脉造影检测下肢血流。结果与正常组同时间点比较,其余各组在术后、治疗7天和28天时双后肢血流差值显著增高(P<0.01);与模型组同时间点比较,各治疗组在治疗7、28天示时双后肢血流差值明显降低,并且联合治疗组在治疗28天时较通心络组和干细胞组降低明显(P<0.05或P<0.01)。与本组术前比较,各治疗组术后、治疗7天和28天时双后肢血流差值比显著升高(P<0.01),但各治疗组治疗7天和28天时双后肢血流差值均较本组术后降低,并且治疗后28天降低更明显(P<0.01)。动脉造影结果显示,模型组和各治疗组均有侧枝循环形成,毛细血管生成,且与已有的毛细血管相互连接形成毛细血管网,其中联合治疗组缺血侧下肢动脉最清晰。结论通心络超微粉联合外周血间充质干细胞移植能明显改善糖尿病大鼠下肢缺血状况,并且优于单纯干细胞移植。Objective To observe the efficacy of Tongxinluo( TXL) ultrafine powder combined with peripheral blood mesenchymal stem cell transplantation in the treatment of lower limb ischemia in diabetic rats. Methods Diabetic rat models were established by high-fat diet combined with low dose of streptozotocin( STZ) injection. Then the right femoral artery and vein were ligated to make the lower limb ischemia models. In total 100 successful models were randomly divided into model group,TXL group,stem cell group,and combined treatment group,with 20 in each. Another 20 healthy rats from the same batch were selected as normal group. The TXL group and combined treatment group received TXL ultrafine powder 0. 4 g /( kg·d) by gavage for 28 days. Three days post-operation,the stem cell group and combined treatment group were injected with 0. 4 ml of cell suspension( PB-MSCs1. 0 × 10^6/ml) at 8 points in the subcutaneous muscle layer of the lower extremity with ischemia,0. 05 ml each point. The normal group,model group and stem cell group received the same volume of normal saline by gavage and were injected with the same volume of sterile RPM1640 medium at the same points in muscles. The laser Doppler perfusion imaging was applied before and after operation,as well as on day 7 and day 28 of treatment,to detect the difference of blood flow between the two hind limbs,and on day 28 of treatment,lower limb angiography was applied to detect the arterial blood flow. Results Compared with the normal group,the differences of blood flow between the two hind limbs in the other groups were significantly larger post-op and on day 7 and day 28 of treatment( P〈0. 01). Compared with the model group,the differences of blood flow between the two hind limbs in each treatment group were significantly smaller on day 7 and day 28 of treatment,and compared with the TXL group and stem cell group,the difference of blood flow between the two hind limbs in the combined treatment group on day 28 of treatment was significantly smaller�
