摘　　要：目的:了解北京市房山区第一医院(以下简称"我院")门、急诊抗菌药物的使用情况,分析抗菌药物使用过程中存在的问题,为临床合理使用抗菌药物提供参考依据。方法:利用医院信息系统(HIS),按比例随机抽取含抗菌药物的门、急诊处方,对抗菌药物使用情况进行点评分析。结果:共抽取含抗菌药物处方1 904张,以口服抗菌药物使用为主,占75.21%。抗菌药物使用以单药为主,占96.01%;二联占3.99%,未发现三联用药。主要使用的抗菌药物种类包括头孢菌素类、氟喹诺酮类和大环内酯类,主要使用的抗菌药物是头孢地尼、头孢呋辛、左氧氟沙星和阿奇霉素。不合理处方占抽取总处方数的13.97%,主要不合理原因包括用法/用量不合理、药物选用不合理、联合用药不合理等。结论:我院门急诊抗菌药物使用情况基本合理,但仍存在不合理之处,需广大医师改进提高。OBJECTIVE：To investigate the use of antibacterial agents in outpatient and emergency department in Beijing Fangshan District First Hospital （our hospital ） analyze the problems in the use of antibacterial agents for reference of clinical rational use of antimicrobial agents. METHODS ： Using HIS system, the antibacterial prescriptions in outpatient and emergency department was sampled randomly for statistical analysis with regard to patients information, use of antimicrobial agents and its rationality etc using Excel table. RESULTS： The oral antibacterial prescriptions showed the highest proportion, accounting for 75.21% of the total antibacterial prescriptions reviewed. The antibacterial agents used singly in 96.01%, in combination of two in 10.09% and in combination of three in none. Cephalosporins, tluoroquinolones and macrolides were the predominantly used antibacterial categories. Cefdinir, cefuroxime, levofloxacin and azithromycin were the most predominantly used antibacterials. 13.97% of the prescriptions were found irrational in CONCLUSIONS： The antibacterial use in from perfect. Physicians need to spare no dosage or administration, drug selection or drug the outpatient and emergency cases was rational on the effort to improve the antibacterial use.
