摘  要:目的评价高频超声心动图与血流动力学检测相结合在阿霉素造模大鼠扩张型心肌病-心力衰竭研究中的价值。方法 30只雄性wister大鼠分为对照组,阿霉素模型组,阿霉素模型组大鼠腹腔注射阿霉素(2mg∕kg体重,1周1次,用药4周后停药观察4周)观察大鼠的一般情况,8周后行高频超声心动图检查及血流动力学指标检测,测定各项心功能指标,检查结束后处死大鼠,取心脏,行HE染色及电镜观察。结果 1与对照组相比较,阿霉素组大鼠心脏的收缩与舒张功能明显下降,表现为:左室收缩末径(LVIDs)及左室舒张末径(LVIDd)明显增加(P<0.0078),左室短轴缩短率FS降低(P<0.036),其差异具有统计学意义。2与对照组相比较,阿霉素组大鼠左心室压(left ventricular pressure,LVP)及左心室内压最大变速率(dp/dt)均明显降低,差异有统计学意义(P<0.0069)。3与对照组相比较,阿霉素组大鼠左心室心肌病理改变和超微结构损伤明显,其差异具有统计学意义(P<0.00612)。结论高频超声心动图可准确评价阿霉素造模大鼠心功能的变化,其结果与血流动力学指标检测结果及心肌组织的病理改变和超微结构损伤高度一致。Objective To evaluate the value of high frequency echocardiography and cardiac hemodynamic examination combined in the study of dilated cardiomyopathy-heart failure of rats caused by adriamycin. Methods 30 male wister rats were randomly assigned to normal control group, adriamycin model group. The adriamycin model group were injected adriamycin to intraperitoneal(2mg/kg, once a week, Withdrawal after 4 weeks, and observed 4 weeks). Observed the rats generally, examined with high frequency echocardiography, cardiac hemodynamics and the left ventricular function after 8 weeks. Then, the rabbits were sacrificed to get the specimens of heart, HE staining and Electron microscopy Observation were applied to the rats'heart. Results 1.The function of Contraction and Diastole of the rats'heart significantly reduced in adriamycin model group than that in normal control group. Reflected as follows: LVEDd and LVEDs significantly increased (P 〈0. 01), and the left ventricular fractional shortening(FS) reduced(P〈0.05), the differences were statistically significant. 2. Left ventricular pressure(LVP) and left ventricular pressure maximum variable rate( dp/dt) significantly decreased in adriamycin model group than that in normal control group, and the differences were statistically significant(P 〈0.01). 3. The pathology changes and ultrastructural damages of myocardial tissues are obvious in adriamycin model group than that in normal control group, and the differences were statistically significant(P 〈0.01). Conclusions High frequency echocardiography can evaluate changes in left ventricular function rats caused by adriamycin accurately, and the rusults of High frequency echocardiography were consistent with the cardiac hemodynamic test results and pathological changes and ultrastructural damages of myocardial tissues.
