经腹部联合经阴道超声对特殊类型异位妊娠的诊断价值

摘　　要：目的探讨彩色多普勒超声对特殊类型异位妊娠的诊断价值。方法对2000~2014年深圳市龙岗中心医院收治的337例异位妊娠的临床资料进行回顾性分析,总结其声像图特征。结果 337例异位妊娠中,特殊类型异位妊娠46例,占13.6%。337例异位妊娠中输卵管妊娠的误诊率1.71%,确诊率98.29%;特殊类型异位妊娠误诊率23.91%,确诊率76.09%。两者的误诊率及确诊率相比有显著差异。结论特殊类型异位妊娠比较少见,对可疑病例应详细询问病史及结合血、尿HCG检查,经腹部合并经阴道超声检查探查,对特殊类型异位妊娠的早期诊断与鉴别诊断具有重要的临床价值。Objective To explore the diagnostic value of color Doppler ultrasound for special types of ectopic pregnancy.Methods Clinical data of 337 patients with ectopic pregnancy who were admitted to our hospital from 2000 to 2014 were retrospectively analyzed. Their characteristics of ultrasonogram were summarized. Results Among the 337 patients with ectopic pregnancy, 46 patients were special types, accounting for 13.6% of the total. The misdiagnosis rate of fallopian pregnancy was 1.71%, and the correct diagnosis rate was 98.29%; the misdiagnosis rate of special types of ectopic pregnancy was 23.91%, and the correct diagnosis rate was 76.09%. The differences of misdiagnosis rate and correct diagnosis rate between the two types of ectopic pregnancy were statistically significant. Conclusion Special types of ectopic pregnancy are rare. Therefore, disease history should be thoroughly asked for suspectable diseases, combined with tests of HCG in blood and urine, and tests of trans-abdominal ultrasonography combined with trans-vaginal ultrasonography, all of which are of great clinical value for early diagnosis and differential diagnosis of special types of ectopic pregnancy.
